کلینک میں مریضوں کی لمبی قطار تھی۔ کوئی حمل کے پہلے چیک اپ کے لیے آئی تھی، کوئی الٹراساؤنڈ کی رپورٹ لیے بیٹھی تھی، اور کوئی کئی سالوں کی بے اولادی کا درد دل میں لیے خاموش تھی۔

اسی دوران ایک خاتون میرے کمرے میں داخل ہوئیں۔

عمر تقریباً 32 سال۔

وہ کرسی پر بیٹھیں، کچھ دیر خاموش رہیں، پھر آہستہ سے بولیں:

“ڈاکٹر صاحبہ… میرے شوہر نے کہا ہے کہ اگر اس بار بھی علاج سے فائدہ نہ ہوا تو وہ دوسری شادی کر لیں گے۔”

کمرے میں چند لمحوں کے لیے خاموشی چھا گئی۔

میں نے ان سے پوچھا:

“آپ کو شادی ہوئے کتنے سال ہوئے ہیں؟”

انہوں نے جواب دیا:

“8 سال۔”

میں نے پوچھا:

“کبھی اپنے شوہر کا بھی ٹیسٹ کروایا؟”

وہ حیران ہو کر میری طرف دیکھنے لگیں۔

“ڈاکٹر صاحبہ… مسئلہ تو میرے اندر ہی ہوگا نا؟”

میں نے کہا:

“ضروری نہیں۔ اولاد نہ ہونے کی وجہ صرف عورت نہیں ہوتی۔ مرد اور عورت دونوں کی جانچ ضروری ہوتی ہے۔”

ہم نے دونوں میاں بیوی کے ٹیسٹ کروائے۔

کچھ دن بعد رپورٹ سامنے آئی۔

مسئلہ عورت میں نہیں تھا۔

مسئلہ شوہر کی رپورٹ میں تھا۔

جب میں نے انہیں یہ بات بتائی تو وہ خاتون خاموشی سے رونے لگیں۔

پھر انہوں نے صرف ایک جملہ کہا:

“ڈاکٹر صاحبہ، کاش مجھے یہ بات 8 سال پہلے معلوم ہوتی۔”

یہ جملہ آج بھی مجھے یاد ہے۔

ہر روز گائناکولوجسٹ کے کلینک میں صرف بیماریوں کا علاج نہیں ہوتا۔

کبھی کسی ماں کی آنکھوں میں پہلی بار اپنے بچے کی دھڑکن دیکھ کر خوشی ہوتی ہے۔

کبھی کسی خاتون کے برسوں پرانے درد کی وجہ سامنے آتی ہے۔

اور کبھی ایک چھوٹی سی معلومات کسی عورت کو برسوں کے الزام اور ذہنی اذیت سے بچا سکتی ہے۔

اگر آپ کے خیال میں بھی بے اولادی کی صورت میں شوہر اور بیوی دونوں کا چیک اپ ضروری ہے تو اس پوسٹ پر “دونوں” لکھیں۔

Reduced fetal movements are not a diagnosis—they are a clinical warning sign that may indicate placental insufficiency, fetal compromise, or an increased risk of stillbirth. Prompt, structured assessment is essential.

✅ Structured history and clinical assessment of RFM
✅ CTG interpretation, fetal behavioural states & sleep cycle
✅ Ultrasound assessment: fetal growth, amniotic fluid volume & Umbilical Artery Doppler
✅ Cerebroplacental Ratio (CPR) and its role in selected pregnancies after 37 weeks
✅ Management of recurrent reduced fetal movements
✅ Timing of birth and when delivery should be considered
✅ Reducing stillbirth risk through timely recognition and evidence-based management

📚 Study these three topics together:
• Fetal Growth Restriction (FGR/SGA)
• Reduced Fetal Movements (RFM)
• Stillbirth (IUFD)

Reduced fetal movements alone are not an indication for delivery. Decisions regarding birth should be based on objective evidence of fetal compromise, placental function, gestational age, maternal condition, and the overall clinical picture.

گزشتہ دنوں میرے پاس ایک خاتون آئیں جن کا بلڈ پریشر 200 تک پہنچا ہوا تھا اور آنکھوں کے آگے اندھیرا آرہا تھا۔جسم پر سوجن تھی اور ایک مرتبہ گھر میں جھٹکا لگ چکا تھا۔

معاملات کی سنگینی کو دیکھتے ہوۓ first aid فراہم کی اور ریفر کردیا۔

اس بیماری کو Eclampsia کہتے ہیں

ایسے high risk case میں آئی- سی- یو کی ضرورت ہوتی ہے جو کہ ہمارے چھوٹے شہروں میں نھی ہوتے ۔خاتون کو جھٹکے لگنے کی وجہ سے ماں بچے کی جان جاسکتی ہے۔

اب وہ خاتون مجھے ملنے آئی، پیاری سی بیٹی پیدا ہوئی جس کا نام مریم رکھا ہے

بتانا یہ چاہتی ہوں کہ جب بھی آپ کا ڈاکٹر آپ کو بڑے شہر ریفر کرتا ہے تو اس کے پیچھے کوئی وجہ ہوتی ہے۔ جبکہ تحصیل ہسپتالوں میں موجود ڈاکٹر کا مزاق اڑایا جاتا ہے کہ یہ صرف ریفر کرنے کے لیے بیٹھے ہیں۔ "اینوں فیصل آباد لے جاؤ اینے نئی بچنا، میمز بنائی جاتی ہیں اور پتا نہیں کیا کچھ کہا جاتا ہے"

LABETALOL IN PREGNANCY — IS IT SAFE OR NOT?

Many people become worried when they hear that a pregnant woman has been prescribed Labetalol. Some even believe all blood pressure medicines are harmful during pregnancy.

Let's clear the confusion.

What is Labetalol?
Labetalol is a medicine used to treat high blood pressure (hypertension). It belongs to a group of medicines called beta-blockers.

Is it safe during pregnancy?
Yes. Labetalol is one of the recommended first-line medicines for treating high blood pressure during pregnancy in many national and international guidelines.

Doctors commonly prescribe it for:
• Chronic hypertension during pregnancy.
• Gestational hypertension.
• Preeclampsia (in appropriate situations).
• Severe high blood pressure in pregnancy.

Why is controlling blood pressure important?
Uncontrolled high blood pressure can be more dangerous than the medicine itself. It can lead to:
• Stroke.
• Placental abruption.
• Kidney damage.
• Reduced blood flow to the baby.
• Premature delivery.
• Maternal and fetal complications.

Can it affect the baby?
When prescribed by a qualified healthcare professional and monitored appropriately, Labetalol has a good safety record in pregnancy.

However, because it crosses the placenta, healthcare providers monitor both mother and baby. Rarely, newborns may experience:
• Temporary low heart rate.
• Temporary low blood sugar.

These effects are usually monitored and managed after delivery if they occur.

Can every pregnant woman take it?
No.

Labetalol may not be suitable for women with:
• Asthma or severe wheezing.
• Certain heart rhythm disorders.
• Very slow heart rate.
• Some other medical conditions.

The Bottom Line

🩺 Placental Tumours

- ✅ Chorioangioma >4 cm carries a high risk of fetal complications.
- ✅ Increased vascularity on Doppler predicts fetal anaemia, FGR, and hydrops.
- ✅ MCA-PSV >1.5 MoM suggests significant fetal anaemia.
- ✅ Differentiate PMD from partial mole using β-hCG and fetal karyotype.
- ✅ PMD is associated with Beckwith–Wiedemann syndrome.
- ✅ Placental metastasis is rare; melanoma is the most frequently reported primary tumour.
- ✅ Fetal interventions should only be performed in specialist fetal medicine centres.
- ✅ Always send the placenta for postnatal histopathological examination.
