Definition

ESR (Erythrocyte Sedimentation Rate) is a hematological test used to measure the rate at which red blood cells (RBCs) settle at the bottom of a vertical tube of anticoagulated blood within one hour.

Principle

When blood is placed in a vertical tube, red blood cells gradually settle due to gravity. The distance they fall in one hour is measured in millimeters (mm/hr). Increased ESR may indicate inflammation, infection, or other diseases.

Objectives

1. To detect inflammation in the body.
2. To assist in diagnosing infections and autoimmune diseases.
3. To monitor the progress of certain diseases.
4. To evaluate the effectiveness of treatment.

Materials Required

- Venous blood sample
- Anticoagulant (Sodium citrate or EDTA)
- Westergren tube
- Westergren stand
- Pipette
- Gloves

Procedure

1. Collect venous blood using an anticoagulant.
2. Fill the Westergren tube up to the marked level.
3. Place the tube vertically in the Westergren stand.
4. Leave undisturbed for 1 hour.
5. Measure the distance the red blood cells have fallen.
6. Record the result in mm/hr.

Observation (Normal Values)

Adult Male| 0–15 mm/hr
Adult Female| 0–20 mm/hr
Children| 0–10 mm/hr

Interpretation

- High ESR → Infection, inflammation, tuberculosis, rheumatoid arthritis, autoimmune diseases
- Low ESR → Polycythemia, sickle cell anemia, severe dehydration
- Normal ESR → No significant inflammatory condition

Advantages

1. Simple and inexpensive test.
2. Useful for detecting inflammation.
3. Helps monitor disease progression.
4. Widely used in clinical laboratories.

Limitations

1. Non-specific test.
2. Cannot identify the exact cause of disease.

Gram staining is a differential staining technique used to classify bacteria into Gram-positive and Gram-negative groups based on differences in their cell wall structure.

Principle

Gram-positive bacteria have a thick peptidoglycan layer and retain the crystal violet-iodine complex, appearing purple. Gram-negative bacteria have a thin peptidoglycan layer and lose the primary stain during decolorization, then take up safranin and appear pink/red.

Objectives

To differentiate bacteria into Gram-positive and Gram-negative groups.

To aid in the identification of bacterial species.

To guide appropriate antimicrobial therapy.

Reagents Used

1. Crystal Violet (Primary stain)

2. Gram's Iodine (Mordant)

3. Alcohol/Acetone (Decolorizer)

4. Safranin (Counterstain)

Procedure

1. Prepare a thin bacterial smear on a clean glass slide.

2. Air dry and heat-fix the smear.

3. Flood with crystal violet for 1 minute and rinse.

4. Add iodine for 1 minute and rinse.

5. Decolorize with alcohol/acetone for 10–20 seconds and rinse.

6. Counterstain with safranin for 30–60 seconds and rinse.

7. Blot dry and examine under an oil immersion microscope (100×).
