What Happens If You Eat One Meal a Day (OMAD) for 30 Days?

One Meal a Day (OMAD) is an advanced form of intermittent fasting where you fast for about 23 hours and consume all your daily nutrition within a 1-hour eating window. While it isn't suitable for everyone, it has attracted growing scientific interest because of its effects on metabolism.

Here's what may happen over the first 30 days:

⏰ 0-8 Hours After Your Meal
Your body digests food, replenishes glycogen stores, and stores excess energy for future use.

⏰ 8-12 Hours
Glycogen stores begin to decline. Your body gradually shifts from using glucose to burning stored fat, and ketone production starts.

⏰ 12-16 Hours
Fat burning becomes more prominent. Ketones increase and begin supplying energy to the brain, which many people report as improved mental clarity and focus.

⏰ 16-23 Hours
Cellular repair processes such as autophagy become more active. During this period, the body starts recycling damaged cellular components, although the extent of autophagy in humans is still an active area of research.

What to Expect During the First Month

Days 1-3
The hardest phase. Hunger, cravings, headaches, fatigue, and irritability are common as your body adapts from relying primarily on glucose to using fat and ketones.

Days 4-7
Most people notice hunger becomes more manageable. Energy levels often improve, and many report better concentration and reduced cravings.

Days 15-30
With consistent adherence and a nutrient-rich diet, potential benefits may include:
• Improved insulin sensitivity
• Reduction in body weight and waist circumference
• Lower triglyceride levels
• Better metabolic flexibility
• Improved mental focus
• Increased fat utilisation

Studies of Ramadan fasting have also demonstrated improvements in several metabolic markers, although results vary depending on diet quality, sleep, hydration, and individual health.

Your One Meal Matters

A single meal should be nutritionally complete and include:
🥩 High-quality protein (fish, eggs, lean meat, legumes)
🥑 Healthy fats
🥬 Plenty of vegetables
🥜 Fibre-rich foods
💧 Adequate fluids and electrolytes

Important Considerations

OMAD is not recommended for everyone. It should be avoided or undertaken only with medical supervision by:
• Pregnant or breastfeeding women
• Children and adolescents
• People with diabetes taking insulin or sulfonylureas
• Underweight individuals
• People with eating disorders
• Those with significant chronic illnesses

Bottom line: Meal timing can influence metabolism, but food quality, total nutrition, physical activity, sleep, and consistency remain the foundations of good health. There is no single eating pattern that suits everyone.

Always consult your physician before starting prolonged fasting, particularly if you have a medical condition or take regular medications.

Teaching a resident to hold an instrument correctly for the first time is a small moment but it's where confidence begins. Every knot tied, every scope passed, every hand steadied under guidance is a step toward the next generation of safe, skilled urologists. Grateful for the opportunity to pass on not just technique, but the discipline and care that comes with it. Proud to be part of shaping the surgeons of tomorrow, one instrument at a time. 🔬🩺

Complications are an inherent part of surgery, even when every decision is evidence-based and every standard of care is followed. The measure of a healthcare system is not whether complications occur, but how they are recognized, managed, and learned from.

Blaming or attacking doctors for every adverse outcome creates fear and discourages surgeons from taking on complex, high-risk cases where patients often have the most to gain. This ultimately harms patient care.

When concerns arise, they should be addressed through due process, transparent investigation, and the rule of law. Accountability is essential, but so is fairness.

Let us support a culture that promotes patient safety, continuous learning, and justice, while protecting the professionals who dedicate their lives to caring for others.

The Styrofoam Cup Theory: The Hidden Trap Behind Every Title

In every profession, titles carry weight. Professor. Consultant. CEO. Director. Chairman. These words command respect, open doors, and often become part of our identity. But there is a hidden danger when we begin to mistake the title for our true worth.

Imagine a simple Styrofoam cup. Fill it with fine coffee, and people admire the coffee—not the cup. Replace it with a crystal mug, and the coffee remains the same. The container may influence first impressions, but it does not change what is inside.

The same principle applies to our careers.

A prestigious designation is only the container. Your knowledge, integrity, competence, humility, and character are the real substance. When we become obsessed with the cup, we risk neglecting what truly matters.

Many professionals spend years chasing promotions, awards, and recognition. While ambition is healthy, titles should be viewed as responsibilities rather than trophies. A promotion does not automatically make someone a better leader. A professorship does not guarantee wisdom. A leadership position does not create respect—consistent actions do.

The most influential people are often remembered not for the titles they held but for the lives they touched. Great teachers are remembered by their students. Great surgeons by their patients. Great leaders by the teams they inspired.

Ironically, Styrofoam itself teaches another lesson. It appears substantial but is lightweight and disposable. Titles can be just as temporary. Organizations change, positions rotate, retirement arrives, and eventually every title is passed to someone else. What remains is your legacy.

So, invest less energy in decorating the cup and more in enriching what it contains.

Continue learning when no one is watching. Treat people with kindness regardless of their status. Share knowledge freely. Lead with humility. Let your work speak louder than your designation.

At the end of your career, people may forget the office you occupied or the title on your business card. They will remember how you made them feel, what you taught them, and the difference you made in their lives.

The Styrofoam Cup Theory reminds us of a simple but profound truth: your title may introduce you, but your character defines you.

Success is not about having the most impressive cup. It is about ensuring that what is inside is worth sharing.

We Are More Biased Than We Think

Every day, we believe we are being objective.

But the truth is, our minds are filled with hidden biases.

We judge people before knowing their story.
We accept information that supports our beliefs and ignore what challenges them.
We praise those we agree with and criticize those we don't.
We remember our successes but justify our failures.

Bias doesn't only affect courtrooms or politics it influences medicine, education, research, leadership, and everyday life.

The first step toward wisdom is not proving you're right. It's asking:

"What if I'm wrong?"

The strongest minds are not those that never change their opinions they are the ones willing to change when the evidence changes.

Stay curious.
Question your assumptions.
Listen before judging.
Let evidence guide your decisions, not emotions or ego.

Awareness of bias is the beginning of better decisions—and better human beings.

😴 You Slept 8 Hours… So Why Are You Still Exhausted?

Many people believe that sleeping longer automatically means sleeping better. In reality, sleep quality is often more important than sleep duration.

During deep sleep, your brain and body perform essential restorative functions. Memories are consolidated, hormones that support tissue repair are released, and metabolic waste products are cleared more efficiently. If your sleep is repeatedly interrupted or of poor quality, you may wake up feeling tired despite spending enough hours in bed.

Here are evidence-based ways to improve your sleep quality:

🌡️ Keep your bedroom cool
A room temperature of 18–20°C (65–68°F) is ideal for most people. A cooler environment helps your body lower its core temperature, an important signal for falling asleep.

🚿 Take a warm shower before bed
A warm shower or bath about 60–90 minutes before bedtime can promote sleep by helping your body cool down afterward.

💡 Reduce light exposure
Dim household lights 2–3 hours before sleep and minimize exposure to bright screens. Blue light from phones, tablets, and computers can suppress melatonin, the hormone that helps regulate sleep.

📱 Keep electronic devices away
Avoid scrolling on your phone in bed. Make your bedroom a place for sleep rather than stimulation.

🍽️ Avoid late-night meals
Try to finish eating at least 2–3 hours before bedtime. Heavy or sugary meals late at night can interfere with restorative sleep and may increase nighttime awakenings.

☕ Limit caffeine
Coffee, tea, energy drinks, and some soft drinks can affect sleep for several hours. If you struggle with sleep, avoid caffeine in the late afternoon and evening.

☀️ Get morning sunlight
Exposure to natural daylight soon after waking helps regulate your circadian rhythm, making it easier to fall asleep at night.

🚶 Stay physically active
Regular exercise and even a daily walk improve sleep quality. However, very intense exercise immediately before bedtime may not suit everyone.

🌬️ Optimize your sleep environment
A quiet, dark, comfortable, and well-ventilated bedroom promotes deeper sleep. Some people also benefit from consistent white noise.

🧘 Practice relaxation
Slow nasal breathing, mindfulness, prayer, or meditation before bedtime can help calm the nervous system and reduce stress.

Remember:

Quality sleep is associated with:
✅ Better concentration and memory
✅ Improved mood and emotional health
✅ Stronger immunity
✅ Better blood sugar regulation
✅ Faster recovery and physical performance
✅ Improved long-term cardiovascular and brain health

If you regularly wake up exhausted despite sleeping 7–9 hours, snore loudly, stop breathing during sleep, or experience excessive daytime sleepiness, consult a healthcare professional. Conditions such as obstructive sleep apnea, restless legs syndrome, thyroid disorders, anxiety, depression, or certain medications may be affecting your sleep.

Don't just aim for more sleep—aim for better sleep. Your health depends on it.

The Scalpel Changes One Life. Teaching Changes Thousands.

Every successful operation is a victory for one patient.

But every resident you teach, every junior doctor you mentor, and every young surgeon you inspire has the potential to save thousands of lives over a lifetime.

A great surgeon is not defined only by technical excellence in the operating room. True greatness lies in sharing knowledge without hesitation, correcting with patience, leading by example, and empowering the next generation to become even better than you.

Medicine advances because experienced hands are willing to guide inexperienced ones. The legacy of a surgeon is not measured by the number of procedures performed, but by the number of skilled, ethical, and compassionate surgeons they help shape.

When we teach, we don't just pass on techniques—we pass on values, judgment, confidence, and a commitment to excellence.

The greatest achievement of any surgeon is to leave behind a generation that serves humanity better than they ever could alone.

👨‍⚕️ Teach generously. Mentor selflessly. Inspire endlessly. That is how true legacies are built.

🍷 **The "Healthy Drinking" Myth Is Finally Over — Here's What the Science Now Says**

For nearly 30 years, many of us grew up hearing that a glass of red wine a day was good for the heart. That belief was built on flawed, bias-ridden studies — and the evidence has now shifted dramatically.

📊 **What the latest research shows:**
A government-commissioned Alcohol Intake and Health Study has confirmed what better-designed research has been signaling for years:

❌ Even **one drink per day** can raise the risk of:
- 🫀 Cardiovascular disease
- 🎗️ Cancer
- 🫁 Liver disease
- ⚰️ Early death

That casual glass of red wine with dinner, the weekend beer, the brunch cocktail — none of these carry any meaningful health benefit. Zero.

🧠 **Why did we get it wrong for so long?**
The old studies were observational — they compared drinkers to non-drinkers, but many non-drinkers in those studies had quit alcohol *because they were already sick.* That made moderate drinkers look healthier by comparison. It was a statistical illusion, not a biological truth.

💬 **The message for patients today:**
The healthiest amount of alcohol is none. This doesn't mean panic or shame for those who drink — it means having honest, informed conversations with patients about updated evidence.

As healthcare providers, our job is to follow the science — even when it overturns comfortable beliefs.
