Laryngopharyngeal Reflux (LPR)
(Silent Reflux)
✅️Definition
Retrograde flow of gastric contents into the larynx and pharynx
▪️Often called "silent reflux" because heartburn may be absent
▪️️Heartburn may be absent
▪️️Pepsin is the major pathogenic factor
▪️️Daytime, upright reflux is more typical than GERD
✅️Most common symptoms:
-Globus sensation
-Throat clearing
-Hoarseness
-Chronic cough
✅️Classic laryngoscopic signs:
-Pseudosulcus vocalis
-Interarytenoid erythema
-Posterior commissure hypertrophy
✅️Best objective test:
- 24-hour dual-probe pH monitoring
-Salivary pepsin positivity strongly supports LPR
