مقعد کے کنارے (a**l margin) پر ایک دردناک، 5 دن پرانا "خود بخود ٹھیک ہونے والا زخم" (self-curing lesion) عام طور پر ایک شدید قسم کے بیرونی بواسیر (acute thrombosed external hemorrhoid) کو ظاہر کرتا ہے جس میں خون جم چکا ہو۔ یہ حالت اکثر اچانک، انتہائی دردناک، زیتون کے سائز کی، اور نیلگوں سوزش (سوجن) کی صورت میں ظاہر ہوتی ہے۔ اسے عام بول چال میں بعض اوقات اس لیے ایسا کہا جاتا ہے کیونکہ یہ کلوٹ (خون کا لوتھڑا) خود بخود ختم ہو سکتا ہے یا جسم چند دنوں سے لے کر چند ہفتوں میں اسے قدرتی طور پر دوبارہ جذب کر سکتا ہے۔
​تاہم، اگر شدید درد 5 دن کے بعد بھی بغیر کسی بہتری کے برقرار رہے، تو طبی معائنہ ضروری ہے تاکہ کسی پھوڑے (abscess)، مقعد کے زخم (a**l fissure)، یا شدید اور نہ ٹھیک ہونے والے کلوٹ (severe unresolving thrombosis) جیسی پیچیدگیوں کو مسترد کیا جا سکے۔
