Post-Streptococcal Reactive Arthritis (PSRA) vs. Acute Rheumatic Fever — Don’t Mix Them Up!

Both conditions can occur after a Group A Strep infection, and both may have positive ASO and anti-DNase B titers with elevated ESR/CRP.

But here’s the key difference 👇

Rheumatic Fever has the classic Jones criteria — including major findings such as carditis, chorea, erythema marginatum, and subcutaneous nodules.

PSRA primarily causes arthritis, but lacks those other major Jones-criteria features. That’s why PSRA is classified separately and does NOT meet the diagnostic criteria for acute rheumatic fever.

Diagnosis starts similarly: evidence of a recent Strep infection + inflammatory markers.

The arthritis in PSRA is non migratory, meaning it stays fixed in the same joints, whereas rheumatic fever's arthritis is migratory, moving from joint to joint.

PSRA also responds poorly to aspirin and anti inflammatory drugs, while rheumatic fever arthritis typically responds fast and dramatically.

No wonder why a child with history of recent strep infection may complain of joint pain for sometimes. Bottom line before treating acute rheumatic fever make sure your diagnosis is correct and save the child years of painful IM penicillin injections. 😊
