Vertebrobasilar Insufficiency (VBI) refers to reduced blood flow through the vertebrobasilar circulation, which supplies important areas of the brain, including the brainstem and cerebellum.

Before performing cervical manipulation, clinicians should consider vascular risk factors and appropriate screening. A negative screening test does not completely rule out vascular pathology, so clinical history, signs/symptoms, and overall risk assessment remain essential.
