Assalam-o-Alaikum,
I hope this message finds you all in good health and high spirits. Today’s Message is about Strengthening Service Delivery, Discipline, and Patient Care.
It has come to my notice that certain operational and administrative aspects within the hospital need immediate attention and improvement. I am writing this communication to all Deputy Medical Superintendents (DMSs) with a clear expectation for enhanced vigilance, discipline, and commitment.
Over the past few days, gaps in supervision, coordination, and reporting have been observed. No comprehensive reporting has been received shift wise regarding the current operational status of the hospital. It is expected that each DMS is fully aware of the ground realities and ensure submission of timely and accurate reporting.
You are directed to strictly monitor and ensure the following:
• Staff Presence & Duty Compliance: Confirm that all staff members by name are present as per their assigned duty rosters at their assigned and are actively performing their responsibilities.
• Strict Observance of Shift Timings: Ensure full compliance with notified shift timings without any deviation in smart attendance as well as biometric attendance.
o Morning Shift: 08:00 AM – 02:00 PM
o Evening Shift: 02:00 PM – 08:00 PM
o Night Shift: 08:00 PM – 08:00 AM
Any late arrival, early departure, or absence must be recorded and dealt with as per rules.
• Strict Observance of Uniform & Dress Code: Ensure that every cadre of staff (Doctors, Nurses, Paramedics, Class-IV, and Security Personnel) is in proper prescribed uniform during duty hours. Non-compliance must be addressed immediately.
• Cleanliness & Hygiene: Conduct thorough inspections of all areas of the hospital, particularly Casualty/Emergency, where patient load is high. Cleanliness must be ensured in wards, corridors, workstations, washrooms, and waste disposal points.
• Casualty Management: Ensure efficient triaging, prompt management of emergency cases, and proper documentation.
• Gynecology & Other Units: Monitor reporting mechanisms, admissions, discharges, and referrals. Ensure that all processes are justified, timely, and in the best interest of patients.
• Clinical Integrity: patient should be properly received examined to their best satisfaction and should be managed properly to the best of your knowledge by writing chief complaint, noting general physical and systemic examination in OPD chit. No patient should be unnecessarily referred outside when all required facilities, human resources, and medicines are available within the hospital. No unnecessary medicines should be prescribed. Any such practice will be viewed seriously.
• Record Keeping & Documentation: Ensure that patient files, histories with DPR and proper intake and output chart, registers, and logs are properly maintained, updated, and readily available for inspection.
• Daily Monitoring & Reporting: Maintain a daily record, including photographic evidence where required, to reflect the actual situation of your assigned areas.
Strict supervision during this initial transitional period is essential. With consistent enforcement, these practices will become routine and institutionalized.
Please understand that this is not personal, it is a matter of collective responsibility. Every patient who comes to our hospital is in distress and seeks relief. It is our professional and moral duty to provide care with dignity, efficiency, and compassion.
Our objective is to ensure that no patient feels the need to seek treatment elsewhere due to lapses within our system. Let us work with sincerity and dedication so that our hospital reflects excellence, discipline, and integrity.
If our intentions are sincere, our efforts will not only fulfill our duty but also serve a noble cause.
You are all directed to ensure immediate and strict compliance.
Thank you.
