Everything That Sounds Logical Is Not Always Physiological

One of the biggest problems in PT is that many ideas sound logical, so people accept them as facts.

Your muscle is tight, so it must be pulled.
Your pelvis is out of alignment.
This technique increases blood flow and heals tissues.
Your spine is out of place
Your pain is because of trigger points. Etc

These explanations often sound convincing, but sounding logical is not the same as being scientifically true.

In discussions, many physiotherapists bring logic, assumptions, and workshop teachings. But where are the evidence, references, and critical thinking?

Healthcare is not built on what sounds good. It is built on what can be tested, measured, and supported by research.
A theory can sound convincing, a technique can look impressive, and a tutor can speak confidently, but none of these automatically make it true.
As physiotherapists, we should be comfortable saying.
I don’t know.
What does the evidence show?
Is there a physiological mechanism behind this claim?
Can this claim be supported by research?

The future of physiotherapy belongs to clinicians who question more, read more, and think critically not those who simply repeat what they were taught.

Many Physical Therapists today are becoming more “Physical” and less “Therapist.” Everything is passive treatment: massage, TENS, ultrasound, hot packs, taping, and manual therapy for almost every patient without proper assessment or clinical reasoning. Neck pain? Hands on. Back pain? Hands on. Knee pain? Same routine again. This is not modern physiotherapy. We are not living in 1995 anymore, so don’t be a dinosaur in healthcare. Modern evidence based physiotherapy is not about keeping patients dependent on treatment tables and machines for months. MT and passive rehab have a role, but they are only tools, not the whole profession. Real rehab is exercise prescription, movement, load management, patient education, communication, reassurance, and critical thinking. Some PTs know many fancy techniques copied from workshops but cannot design a proper strengthening program or explain pain scientifically to patients. That is dangerous. Physiotherapists should stop creating fear and dependency by making patients believe they are permanently misaligned or fragile. The goal of rehab is not to make patients addicted to passive care, but to make them stronger, confident, functional, and independent. The future belongs to clinicians who follow science and think clinically, not dinosaurs still surviving on ultrasound and hot packs.

Trigger Points has become one of the most overused and abused diagnoses in physiotherapy. Many patients daily come saying, I have trigger points, because someone already blamed all their pain on this so called universal problem. In many clinics, when physiotherapists do not truly understand the patient’s condition, biomechanics, loading pattern, psychosocial factors, or actual diagnosis, they simply say: It’s trigger points bro. Modern research still debates the exact existence, reliability, and diagnostic accuracy of trigger points. Studies show poor inter examiner reliability, meaning two clinicians often cannot even agree if a trigger point is really present. Pain is far more complex than pressing painful spots in muscles. Trigger points should not be treated like a magic diagnosis explaining every neck pain, back pain, headache, or body ache. Real evidence based physiotherapy is not about blindly releasing knots for temporary relief, it is about understanding the patient, movement, loading, strength, lifestyle, nervous system sensitivity, stress, and long term rehab. Trigger points may exist as part of pain presentation, but they are not the answer to every human problem. A strong clinician investigates deeper instead of hiding weak assessment skills behind fashionable terminology.

Not every physiotherapy workshop is bad.
But let’s be honest, in Pakistan, the majority have become more business than education.

Many so called expert tutors are not even treating real patients regularly in clinics.
Some failed in clinical practice, so now they are selling physiotherapy in seminar halls with big claims, fancy marketing, and printed certificates.

A 2 day workshop can’t make someone an expert.

Real physiotherapy is built through,
Patient handling, clinical reasoning, EBP, failures, experience, and years of learning.

But today, many young physiotherapists are being trapped by workshop culture:
Learn this technique and become specialist instantly.

This is dangerous.

Teaching random protocols without proper assessment and reasoning creates overconfident but incompetent physiotherapists.

Healthcare is not a stage show.
Patients are not experiments for half trained clinicians who only know techniques copied from workshops.

The sad reality is:
Some tutors are better at marketing themselves than actually treating patients.

Again, not every tutor is bad.
There are still genuine mentors teaching with honesty and science.

But the fraud workshop culture in Pakistan is damaging the profession badly.

Young physiotherapists should stop chasing certificates like trophies and start focusing on competence, science, communication, exercise prescription, and real clinical skills.

Because in the end, patients do not care about your seminar pictures.
They care whether you can actually help them recover.
