What Type of Menstrual Pain Should You Not Ignore?

Menstrual cramps are common and are often described as cramping or a dull ache in the lower abdomen before or at the beginning of menstruation. The pain may also radiate to the lower back or thighs.

Menstrual pain can result from uterine contractions without any underlying medical condition. However, if the pain interferes with daily activities, becomes progressively worse, or is accompanied by unusual symptoms, you should see a doctor to determine the cause. You do not have to endure severe pain every month.

5 Warning Signs That You Should See a doctor

1. Pain that interferes with daily life: You have to miss school or work, lie down because you cannot carry out normal activities, or continue to experience pain despite taking pain medication appropriately.

2. Changes in the pattern of pain: Your menstrual pain becomes increasingly severe with each cycle, or you begin experiencing significant pain despite not having had similar symptoms before.

3. Heavy or abnormal menstrual bleeding: Examples include soaking through a sanitary pad every 1–2 hours, having periods that last longer than 7 days, passing large blood clots, or experiencing bleeding between periods.

4. Pain outside your menstrual period or pain that continues after your period ends: This is particularly concerning when the pain recurs or interferes with your daily activities.

5. Pain during urination, bowel movements, or sexual in*******se: This is especially important when the symptoms recur or become more pronounced during menstruation.

How Can You Manage Menstrual Pain at Home?

- Apply heat to your lower abdomen. Use a warm compress or a hot water bottle wrapped in a towel, and avoid excessive heat to prevent burns.
- Get enough rest and engage in gentle physical activity. Activities such as walking, stretching, or yoga, as tolerated, may help relieve menstrual pain.
- Keep a menstrual symptom diary. Record the start and end dates of your period, pain intensity, menstrual flow, associated symptoms, medications taken, and how well they work.
This information can help your doctor assess your condition.
- Use pain relief medication appropriately. Nonsteroidal anti-inflammatory drugs (NSAIDs), such as ibuprofen, help reduce the production of prostaglandins, substances involved in uterine contractions. This can help relieve menstrual cramps. These medications often work best when taken at the onset of pain or menstruation. Follow the product label or your doctor’s or pharmacist’s advice, and do not take multiple NSAIDs together.

If you have a history of allergies to these medications, stomach ulcers or gastrointestinal bleeding, kidney disease, take anticoagulants (blood thinners), or may be pregnant, consult a doctor or pharmacist before using them.

- Seek medical attention if the pain affects your daily life. If the pain is more severe than usual and medication does not help, seek prompt medical evaluation.

When Should You Seek Emergency Medical Care?

- Sudden, severe lower abdominal pain, particularly one-sided pain, or pain accompanied by dizziness, fainting, difficulty breathing, or heavy bleeding.
- Lower abdominal pain accompanied by fever, vomiting, or abnormal vaginal discharge. These symptoms warrant prompt medical attention.

If your period is late or you could be pregnant and experience lower abdominal pain or vaginal bleeding, seek urgent medical evaluation. The bleeding may not be a normal menstrual period and could be associated with an ectopic pregnancy, a potentially life-threatening condition in which a pregnancy develops outside the uterus.

Severe Menstrual Pain May Be Caused by Treatable Conditions

Severe menstrual pain may be associated with conditions such as:

- Endometriosis: A condition in which tissue similar to the lining of the uterus grows outside the uterus.
- Adenomyosis: A condition in which tissue similar to the uterine lining grows into the muscular wall of the uterus.
- Uterine fibroids: Noncancerous growths in the uterus.
- Pelvic inflammatory disease (PID): An infection of the female reproductive organs.

Certain ovarian cysts can also cause lower abdominal pain, although the pain is not necessarily limited to menstruation.

These symptoms alone cannot confirm any specific diagnosis. Your doctor will assess your medical history and symptoms, perform an examination when appropriate, and may recommend additional investigations, such as an ultrasound, depending on your condition.

Do not let menstrual pain become something you feel you must endure every month. If the pain affects your quality of life, consult a doctor or gynecologist to identify the underlying cause and receive appropriate treatment.

(Recurrence Score)

“Can I Choose Not to Have Chemotherapy?” A Question Many Breast Cancer Patients Ask Themselves

Many patients diagnosed with breast cancer feel anxious about chemotherapy, particularly its potential side effects, fatigue, and hair loss. These concerns often lead them to wonder, “Can I choose not to have chemotherapy?”

These concerns are completely understandable. However, before making a decision, one of the most important questions to ask yourself and your treating physician is: “How much benefit would chemotherapy provide in my specific case?”

Because chemotherapy may not be necessary for everyone.

- For some types of breast cancer, chemotherapy can significantly reduce the risk of recurrence.
- For others, hormone therapy alone may be sufficient.

So, how can you determine which group you belong to?

Introducing the HapOnco Breast 21 Test

The HapOnco Breast 21 test is designed for patients with early-stage, hormone receptor-positive (HR+) and HER2-negative (HER2−) breast cancer.

The test does not require an additional invasive procedure, as it uses the existing tumor tissue collected during surgery and stored at the hospital for analysis.

The test report provides three important pieces of information to help guide treatment planning:

1. Recurrence Score: An assessment of the risk of cancer recurrence.

2. 10-Year Recurrence Risk: The estimated percentage risk of the cancer returning within 10 years.

3. Chemotherapy Guidance: Information to help determine whether chemotherapy may be beneficial.

Understanding the Test Results

- Low Risk (Low Score): Hormone therapy alone may be sufficient for some patients, helping them avoid chemotherapy when it is unlikely to provide meaningful additional benefit.
- High Risk (High Score): Chemotherapy may offer a significant benefit in reducing the risk of recurrence.
- Intermediate Risk (Moderate Score): The physician will consider additional factors, such as the patient's age and menopausal status, when determining the most appropriate treatment.

Once the results are available, the physician will interpret them alongside other important factors, including tumor size, disease stage, and individual patient characteristics, to develop the most appropriate treatment plan.

This approach may help some patients avoid unnecessary chemotherapy while giving others greater confidence that chemotherapy is a worthwhile and appropriate option for their individual circumstances.

Important Note: Treatment decisions should be made by the treating physician based on clinical judgment and each patient's individual condition. Test results are intended to support clinical decision-making and should not be used as the sole basis for determining treatment.

Why Can You Still Catch a Cold After Getting the Flu Vaccine?

Have you ever wondered why you can still experience coughing, sneezing, a runny nose, or fever even after receiving the influenza vaccine?

The truth is that the influenza vaccine does not protect against all respiratory infections, nor does it provide 100% protection against influenza. Its effectiveness can vary depending on several factors. After vaccination, the body needs approximately 2 weeks to develop sufficient immunity to help protect against the virus. Therefore, you may still be susceptible to infection during this period.

Although the vaccine cannot prevent every infection, it helps reduce the risk of influenza and lowers the likelihood of severe illness and complications.

What Are the Possible Reasons?

1. You may be infected with another type of virus:
Cold-like symptoms can be caused by various viruses, such as rhinoviruses or the virus that causes COVID-19. The influenza vaccine is not designed to protect against these viruses.
2. Your body needs time to build immunity: After vaccination, the body needs approximately 2 weeks to develop an immune response. During this period, you may still be susceptible to infection.
3. Vaccine effectiveness varies: The effectiveness of the vaccine depends on how well the virus strains included in the vaccine match those circulating in the community, as well as factors such as age and overall health.
4. The vaccine helps reduce the severity of illness: Even if you still become infected, vaccination can help reduce the risk of severe illness and complications caused by influenza.

How Should You Take Care of Yourself After Vaccination?

- Wash your hands regularly.
- Avoid close contact with people who are sick.
- Wear a face mask in crowded places or when exposed to higher-risk environments.
- Get enough rest and eat a balanced, nutritious diet.
- Follow healthcare professionals’ recommendations regarding vaccination.

Why Do You Need a Flu Vaccine Every Year?

Influenza viruses constantly change, and immunity from vaccination may decrease over time. Therefore, annual influenza vaccination is recommended to help ensure protection against the strains expected to circulate during each flu season.

In conclusion, you can still get sick after receiving the influenza vaccine. However, vaccination remains an important way to reduce the risk of severe illness and complications. Remember to take good care of your health alongside vaccination.

If you experience difficulty breathing, chest pain, confusion or decreased alertness, or other severe symptoms, seek medical attention immediately.

(Septicemic Plague)

Plague: How Is It Transmitted and What Symptoms Require Urgent Medical Attention?

Plague is an infectious disease caused by the bacterium _Yersinia pestis_, which naturally occurs in rodents such as rats and squirrels. Fleas are an important vector for transmission. Although plague is now rare, it is a serious disease with a relatively short incubation period of approximately 1–7 days after exposure.

How Is Plague Transmitted?

There are three main ways people can become infected:

1. Being bitten by an infected flea
This is the most common route of transmission. A flea can become infected after feeding on an infected animal and then transmit the bacteria when it bites a person.

2. Direct contact with an infected animal
Infection can occur through contact with the blood, tissues, or bodily fluids of an infected or dead animal, particularly without appropriate protective equipment.

3. Respiratory droplets
This can occur when a person with pneumonic plague coughs or sneezes at close range. This is the form of plague that can be transmitted directly from person to person.

Key Symptoms to Watch For

Symptoms vary depending on how the bacteria enter the body.

1. Bubonic Plague — The Most Common Form
Common symptoms include:
- Sudden high fever and chills
- Severe headache
- Extreme weakness and fatigue
- Swollen and very painful lymph nodes, known as buboes, usually occurring in the groin, armpits, or neck

2. Pneumonic Plague — Severe and Rapidly Progressive
Symptoms may include:
- High fever
- Cough
- Chest pain
- Shortness of breath or difficulty breathing
- Sputum that may contain blood

Pneumonic plague is particularly dangerous because it can spread from person to person through respiratory droplets and can become life-threatening without prompt antibiotic treatment.

3. Septicemic Plague
This occurs when the bacteria enter the bloodstream, either directly or as a complication of bubonic or pneumonic plague.

Symptoms may include:
- High fever
- Abdominal pain
- Diarrhea
- Shock
- Bleeding under the skin
- Severe damage to tissues, which may cause the fingers, toes, or other extremities to become dark or black due to loss of blood supply

Can Plague Be Treated?
Yes. Plague can be treated with antibiotics, such as streptomycin, gentamicin, or doxycycline. However, early treatment is critical, particularly in cases of pneumonic plague.

If you develop symptoms that may be consistent with plague—especially after contact with a sick or dead animal or travel to an area where plague is occurring—seek medical attention promptly and inform your healthcare provider about your exposure history.

How Can Plague Be Prevented?
- Avoid contact with wild animals and rodents, especially animals that appear sick or are found dead unexpectedly.
- Wear protective gloves if you must handle animals or animal carcasses.
- Prevent and control flea infestations in pets and around the home. Use appropriate insect repellent when entering areas where exposure to fleas may occur.
- Maintain good sanitation: Keep food and garbage in tightly sealed containers to avoid attracting rodents.
- Tell your doctor about any potential exposure: If you develop a fever after traveling to an area where plague occurs or after contact with rodents or other potentially infected animals, inform your healthcare provider immediately.

Although plague is uncommon today, it remains a serious infectious disease. Knowing how it spreads and recognizing the warning signs early can help ensure timely treatment and reduce the risk of serious complications.

Wading Through Floodwater: What Diseases Should You Watch Out for?

Floodwater poses more than just a risk of drowning. It may also be contaminated with bacteria, chemicals, sewage, and other harmful substances. The risk can be higher when you have to walk through floodwater for an extended period of time.

1. Leptospirosis (Leptospira infection)
Leptospirosis is caused by bacteria that may be present in contaminated water or soil. The bacteria can enter the body through cuts, wounds, or abrasions on the skin. Common symptoms include high fever, headache, muscle aches, and red eyes.

2. Skin Infections and Infected Wounds
Prolonged exposure to water can cause the skin to become soft and more prone to cuts and wounds. Contact with contaminated floodwater may increase the risk of infection, especially when there are open wounds.

3. Diarrhea and Food Poisoning
Floodwater may be contaminated with bacteria and other pathogens from sewage and waste. Consuming contaminated food or water can cause diarrhea, vomiting, abdominal pain, and other gastrointestinal symptoms.

4. Conjunctivitis (Pink Eye)
Contact with contaminated water may increase the risk of eye infections, which can cause redness, irritation, burning, or discharge from the eyes.

5. Respiratory Illnesses
After the floodwater recedes, moisture, mold, and dust generated during cleaning may trigger coughing, sneezing, or respiratory irritation, particularly in people with pre-existing respiratory conditions.

6. Injuries from Sharp Objects and Debris Underwater
Floodwater can make it difficult to see what is beneath the surface. You may accidentally step on broken glass, nails, metal objects, or other debris, resulting in cuts and increasing the risk of infection.

What Should You Do After Wading Through Floodwater?

- Shower and wash your feet thoroughly with clean water and soap.
- Clean and disinfect any wounds as soon as possible.
- Avoid consuming food or drinking water that may have been contaminated.

Lynch Syndrome

Some Cancers Can Be “Inherited” from Parents to Their Children

When we think of cancer, we may assume that it is mainly caused by lifestyle factors. However, genetic factors can also play an important role in the risk of developing certain types of cancer.

Most cancers are not directly inherited. However, approximately 5–10% of all cancers are associated with inherited genetic mutations passed from parents to their children. This is known as hereditary cancer.

What Does “Inherited” Mean?

Our genes play an important role in controlling cell growth, cell division, and DNA repair. If a person inherits a harmful mutation in one of these genes from a parent, their cells may have an increased risk of developing certain cancers.

However, having a cancer-related gene mutation does not mean that a person will definitely develop cancer. It means that their risk may be higher than that of the general population. Knowing this risk can help guide appropriate screening and health monitoring.

Common Cancers and Hereditary Conditions

- Breast and ovarian cancer: Often associated with mutations in the BRCA1 or BRCA2 genes.
- Colorectal and endometrial cancer: May be associated with Lynch syndrome.
- Prostate and pancreatic cancer: In some families, the risk may be linked to genes that are also associated with hereditary breast and ovarian cancer.

Family History Warning Signs to Look Out For

- Several close family members, such as parents, siblings, or children, have had the same or related types of cancer.
- Cancer was diagnosed at a relatively young age, such as breast or colorectal cancer before age 50.
- One family member has had more than one type of cancer or cancer affecting both paired organs, such as cancer in both breasts.
- A rare type of cancer occurs in a family member, such as male breast cancer.

How Can You Take Action?

If your family history includes any of these warning signs, discussing your family history with a doctor or genetic health professional can help determine the most appropriate next steps.

Depending on the individual and family history, genetic testing may be considered to assess inherited cancer risk and help develop a personalized screening and monitoring plan.

Understanding your family history and genetic risk is not about creating fear. It is about gaining information that can help you take preventive action and make more informed decisions about your health.

Know your genes. Know your risk. Plan your health with greater confidence.
