The uncertainty of family caregiving can be as exhausting as the work itself. You do not know whether you are planning your life around months, years, or decades of care. Major decisions get delayed, plans remain tentative, and the future becomes difficult to imagine. You keep living, but so much of your own life remains on hold because caregiving has no predictable timeline.

HARD FACTS.
If your loved one is already dying, what are you asking us to bring them back to?

That may be one of the hardest questions I ever ask a family.

But it may also be one of the most important.

When someone has a terminal illness, their body is becoming increasingly frail, and they are approaching the end of life, eventually their heart will stop.

And in that moment, families sometimes cry:

“Do everything.”

“Do whatever it takes to keep them alive.”

Those words come from love.

But if your loved one is on hospice, I want you to understand what “everything” can actually mean.

CPR is not gentle.

Chest compressions must be forceful enough to circulate blood when the heart no longer can. Ribs or the breastbone can fracture. A breathing tube may be placed. Medications may be given. Electrical shocks may be used when the heart rhythm is one that can be shocked.

Sometimes we can restart the heart.

But here's the part I desperately want families to understand:

Restarting the heart does not restart a body that is dying from terminal disease.

CPR cannot cure the cancer.

It cannot reverse advanced dementia.

It cannot restore organs that are failing.

It cannot erase the disease that brought someone to hospice.

And even if circulation returns, a very frail or seriously ill person may never return to their previous level of functioning.

They may require a ventilator.

They may suffer brain injury.

They may require intensive hospital care.

And despite everything that was done…

they may still die.

This is why conversations about DNR orders are so important in hospice.

A DNR does NOT mean:

“Do nothing.”

It means something very specific:

If my heart stops or I stop breathing, do not attempt CPR. Allow my natural death to occur.

And until that moment?

We care for you.

We treat your pain.

We treat your shortness of breath.

We treat anxiety and agitation.

We reposition you.

We bathe you.

We protect your dignity.

We support your family.

We sit beside you.

We hold your hand.

A DNR does not mean we stop caring.

Sometimes it means we stop trying to prevent the inevitable and put all of our energy into making sure someone is comfortable while it happens.

Choosing a DNR is personal, and being on hospice does not automatically mean someone must have one. You deserve to understand your options and discuss them with your hospice team and healthcare provider.

But please don't wait until your loved one's heart has stopped to begin thinking about this.

Ask the harder question now:

If CPR could restart their heart but could not restore the life or function they had before, would they want it?

Not what do you want because you're afraid to lose them.

What would THEY want?

That is love, too.
