We usually offer both options: one that is covered by insurance (the standard prescription), and a compounded version that typically is not covered.

So why do we often lean toward the compounded one? It comes down to what is not in it. Compounded vaginal estrogen is made without many of the additives and buffers found in the classic prescription, and those additives can be a hidden source of irritation. Patients often report less irritation and fewer side effects as a result.

That matters a lot when we are trying to treat the whole person. If you are already dealing with frequent dysuria (burning with urination), the last thing you want is a product that adds to the irritation. Removing those unnecessary buffers can make a real difference.

D-mannose for UTI prevention: how is it supposed to work, and does it actually? The science is still unfolding. ๐Ÿ’š

Here is the theory. D-mannose is a sugar that humans excrete in its intact form, meaning when you consume it, the liver and kidneys do not change it. It shows up in the bladder exactly as you took it.

Once there, it is thought to bind to the pili, the tiny hair-like structures that bacteria such as E. coli and Klebsiella use to attach to the bladder wall. By latching onto those pili, D-mannose may stop the bacteria from gripping on, and help prevent infection.

That is the idea. But here is where it gets interesting: the MERIT trial in the UK seemed to challenge it.

So is MERIT the final word? Not necessarily. We recently sat down with Dr. Philippe Zimmer, who is running an ongoing trial to dig deeper into whether D-mannose truly prevents UTIs through this mechanism.

The honest takeaway: D-mannose is popular and low-risk, but the evidence is still being written. And that is exactly why good research matters.

Boric acid, or clindamycin, or metronidazole? For BV, the right choice really comes down to you.

A big part of the decision is simple: how much are you suffering right now?

If your symptoms are worse than a 5 out of 10, a topical antibiotic like metronidazole or clindamycin may be the better fit. They take a broader-spectrum approach, targeting the bacteria likely driving your symptoms.

If your symptoms are milder, under that 5 out of 10, boric acid may be worth considering. It is not an antibiotic, it is an antiseptic, and it can offer similar results with a gentler approach.

And they are not always either-or. In some cases, boric acid can be combined with topical metronidazole or clindamycin. It all depends on your specific situation, which is exactly why this should be assessed case by case.

One more thing worth knowing: if your symptoms keep coming back, it is important to be evaluated for aerobic vaginitis, which can be quietly contributing and is often missed.

Diagnosed with bacterial vaginosis? Here's something important to know: there is no one-size-fits-all treatment.

BV is common, but the way it is treated should still be personal. A few options your provider might discuss:

Boric acid
Topical metronidazole
Topical clindamycin
Or an oral pill
