Facility staff sometimes expect the same format from every visit, but medication management and supportive therapy serve different purposes. One reviews symptoms, side effects, and dosage. The other focuses on coping and day-to-day adjustment.

Before adjusting a medication for sundowning, we look at the environment and routine around the resident first: lighting, noise, shift changes, and disruptions to a normal day. The symptom alone rarely tells the full story.

Medication-assisted treatment isn't a single option applied the same way to everyone. Suboxone and Vivitrol serve different points in a person's stability and treatment history, and the choice between them is a clinical decision, not a preference. In a facility setting, that decision is made alongside staff who see the resident daily.

Multiple prescribers, changing dosages, and new additions from a hospital stay can all interact in ways that show up as a mood or behavior change.
