We are currently seeking a dynamic and customer service oriented individual to join our team as our SNF/LTC Nurse Manager.

Qualified candidate will have the ability to lead, guide and direct overall operations and coordination of care as directed by DNS. This role works collaboratively with staff, physicians and other professionals to ensure the best possible outcome for residents. Supports the DNS to deliver high quality of care and assisting with staff and resident management.

Excellent communication and leadership skills required and previous long term care management experience preferred.

It has been a privilege to know and care for Connie over the years, and she will be deeply missed by our residents, staff, and everyone whose life she touched. We extend our heartfelt condolences to Connie’s family and loved ones during this difficult time ❤️

Constance was born on August 14th, 1935 and passed away on July 13th, 2026 at the age of 90
