"Dysphagia doesn't discriminate. It affects everyone from premature infants to the elderly. However, the causes of dysphagia define the treatment. Performing a good comprehensive dysphagia evaluation is the first step in treatment, as the medical team needs to figure out what are the underlying causes of this symptom known as dysphagia. Then, the foundation of dysphagia treatment/management is a person-centered care approach. You are at the center of your medical team. You should review findings with your medical team and seek answers to all your questions before making informed decisions about your care."

This year’s very brief highlights for me:

You get actual reference values from Dr Steele for your videofluoroscopic swallow studies to know what is really a delay (for example).

Dr Molfenter provides great explanations of sarcopenia, homeostenosis, and how she proved this happens in the pharyngeal muscles. She shares exciting new research on proactive exercises to hold back the muscle breakdown with aging. She shared great Frailty scales too.

The SLP Amber Heape shared crucial skilled nursing facility perspective on person centered care and engaging the whole person in their medical team making informed decisions and “informed refusals!” Just don’t call someone non-compliant. The SLP is not the one to tell someone what they are “allowed” to eat or not. “Dysphagia management is rarely a risk free option.” Balance what may be safer with the person’s wishes and quality of life. This person centered approach and Good documentation and good team conversations can help avoid fears of litigation (as I showed in my Medbridge talk with Dr McCoy!)

Lastly ENT, Dr Abu-Ghanem shared the ENT “workup” for age related normal changes to physiology (primary presbyphagia) versus primary or secondary sarcopenia. Pushing for oropharyngeal dysphagia (beyond that of normal aging) to be considered a “geriatric syndrome.”
