A new study is adding another layer to the conversation around hormone replacement therapy, menopause and brain health.

Researchers looking at more than 183,000 postmenopausal women found that HRT use was associated with a lower risk of all-cause dementia overall, with stronger associations seen in certain groups of women.

But this is where the nuance matters: this was an observational study, which means it can show an association, not prove that hormone therapy prevents dementia.

What it does tell us is that the relationship between estrogen, menopause timing, HRT and cognitive health deserves more attention. Timing of treatment, surgical menopause, APOE ε4 status, lifetime estrogen exposure, cardiovascular risk, personal history and the type of hormone therapy may all matter when evaluating whether HRT is appropriate for an individual patient.

Hormone replacement therapy should never be treated as a one-size-fits-all longevity tool or a guaranteed dementia-prevention strategy. The decision should be based on the full picture: your symptoms, age, timing of menopause, medical history, family history, cardiovascular health, clotting risk, cancer risk and personal goals.

This is exactly why women’s health, perimenopause, menopause, hormone optimization and healthy aging need more individualized conversations.
