📝 TWO PHRASES EVERY U.S. NURSE SHOULD KNOW
👩⚕️ Yesterday's nursing wisdom:
"If it wasn't documented, it wasn't done."
💻 Today's nursing reminder:
"If it's documented... was it really done?"
Both statements matter.
The first reminds us that documentation is our professional responsibility.
The second reminds us that documentation must be truthful, timely, accurate, and reflect the care we actually provided.
In today's healthcare environment, clicking a box is not the same as assessing a patient.
Templates don't replace critical thinking.
Auto-filled notes don't replace professional judgment.
Your documentation tells the patient's story...
Make sure it's THEIR story—not yesterday's.
