I am always forgetting the porphyrias! Providers with tricky cases, don’t forget to think about them!

I almost did again today. But then I remembered the porphyrias, and solved all the world’s problems.

Well, at least solved one of my patient’s problems 🤓

And then I made these obnoxious stickers to plaster all over my office so I stop being silly and forgetting to consider the porphyrias as differential diagnosis is complex cases.

The best thing about hEDS is sometimes when things are good you can convince yourself you don’t have it. Maybe I’m fine, maybe everyone was wrong, maybe I just need to stop whining and saddle up.

And then you get your period right around the same time as a deep tissue massage and several rounds of using your bendy body to chase chickens through brambles and it’s a perfect storm of crap that reminds you that your perfectly orchestrated performance of this disease day in and day out is really jsut a house of cards that can come tumbling down under the right circumstances.

So here we are. Shoulder capsule and ribs already put back in once, knees buckling, left ankle flaring, neck not really holding on by much, literally. And of course it’s FREAKING HOT so I’m suited up on my finest Jelliebend with my ice necklace and keeping the propranolol and licorice tincture nearby.

Yeah. This is a thing I have. Thankful for the good times, happy to have backup plans, support and the ability to rest for the bad.

Seeing all you fighters out there dealing with the same crap pile - you have to be so incredibly strong to deal with a disease that makes you feel so weak, and I see your bravery 🫡👊❤️

Goddamn subscap.

Let’s talk this through though. This is my experience, which of course may not be yours, but here’s why this is happening to me. One of the places where my ligaments really don’t seem to be ligamenting well is my left scapula. As a result, I have scapular dyskinesis with mild winging (ya know, that super awesome clunking sound every time you roll the shoulder back). Because human bodies tend to have underdeveloped stabilizer muscles because we largely rely on ligament stability, my rotator cuff, and my left subscapularis in particular, tries like all hell to step in and do the stabilizing work.

Buuuut it’s not that good at it. Our rotator cuff are what I call “effector” muscles- muscles that move joints, not keep them in place. But now it’s working super hard to do both, meaning I have both serious muscle fatigue around my left scapula and also a lot of muscle spasm, pain and general unhappiness.

It’s also destabilizing my AC, joint - our proper shoulder joint that attaches our shoulder to our clavicle - because the anterior (from side) of the subscap attaches to that joint and when it’s spasming, it pulls it forward. Not to mention that when your scapula wings it’s actually rolling forward, causing a domino effect that “dumps” your shoulder joint forward.

Lots of ouch.

The solution, like most issues in the joints that have their root in the loss of ligament support, is to bring up those stabilizers. They have to kick on now - they have to be a reliable plan B in the case of ligament failure. They have to make themselves into an internal brace so that the ligaments can fail safely without the joint destabilizing, and without the effector muscles trying to do the job instead.

It’s work, and it’s not happening today. Today I sling the arm and let it rest. Tomorrow, I keep bringing up those scapular stabilizers that need to get stronger (which for me is primarily the anterior serratus) with a variety of stabilization exercises.

Seriously, f*%k EDS. But there’s help.

Also not me considering going back for my DPT instead of finishing my DNP 🤓

It’s not that the rarer stuff doesn’t happen. It’s just, well, rarer! And it shouldn’t be the first thing providers reach for at the expense of ignoring the very low-hanging fruit that is severe dysfunction of the musculoskeletal system and the nerves that around it. If you’re diagnosing someone with a “surgery-only” condition when they have never seen hypermobility-specific PT or build stabilizer strength, or always seem to be blaming someone’s symptom cluster on whatever is hot on social media right now, youuurree probably doing it wrong. Or trying to make a buck.

I’ve said this before and I’ll say it again. hEDS is not flashy. It’s boring, dreadful, takes a ton of work and real progress takes time. As someone who also treats the folks who are suffering from debilitating conditions related to this disease that do need surgery, I can tell you with 100% certainty, they did not end up with their diagnosis lightly.

And none of this applies to conditions of the viscera or brain.

I feel like I have to put a ton of disclaimers on this post because EDS Tik-Tok is coming for me lol, but it’s important to say. Avoiding misdiagnosis in a syndrome as complex as hEDS means doing more than asking your patient to fill out a form or see if your latest Reel resonates. Teasing through this muck takes time, diligence, experience and the acceptance of that fact that most of the time, peoples joints, muscles and nerves just aren’t sitting right.

It was a year ago to the day today that we moved into our current home in Homer. I have no idea how time passed so quickly, and we are rounding out the last season of surprise treasures in flora around the property.

Last year, the poultry run was still enclosed with old wire, but we tore the wire down at the end of Fall and this weekend new wire goes up to make space for chickens of our own to live and play. But not before amazing discoveries this past month about the plethora of medicinal plants I couldn’t see last year with the run still enclosed.

Nettle is peeking here, almost totally covered by other friends.

And of course, St. John’s Wort, which is all over our property but I didnt know was in here too. He and I follow each other everywhere, I’m a Leo with big solar energy that peaks at Summer Solstice so this is a surprise to no one lol.
