RE+FORM Physical Therapy and Wellness offers outpatient physical therapy and wellness sessions to assist in the management of musculoskeletal pain, post-operative conditions, and positional vertigo. RE+FORM Physical Therapy and Wellness offers outpatient physical therapy (clinic or house calls) and wellness sessions to assist in the management of musculoskeletal pain, post-operative conditions, and positional vertigo. WHY CHOOSE RE+FORM?

1) You will receive 1:1 undivided attention and quality treatment techniques rendered by a licensed board-certified physical therapist and certified wellness specialist. No use of rehab technicians and/or students.

2) We will tailor a program for you if finances, time, logistics, and current physical state may be a limitation.

3) We will bring all the necessary exercise equipment and skillset to assist in your recovery.

4) Mobile outpatient PT "house calls" and wellness sessions can be performed in the comfort of your home/office/gym at a time that is convenient for you. No need to waste time driving to/from a clinic, fight traffic, rush to leave work, or hire a babysitter for childcare.

5) We are a certified Medicare Part B provider and take most commercial insurances. We will verify your outpatient PT benefits, let you know exactly how much your session will cost, collect your copay/coinsurance (if there is one), and submit your claim to your insurance company. You will never receive a surprise bill in the mail from us - GUARANTEED!
