A new 2026 review proposes something called the hormone–tissue–load mismatch. During the menopause transition, hormonal changes can affect: • bone remodeling and microarchitecture • collagen turnover and tendon repair • muscle mass and strength • ligament compliance • neuromuscular control That mismatch between hormones + tissue capacity + mechanical load may help explain why some women suddenly develop problems like Achilles or rotator cuff tendinopathy, plantar heel pain, or stress injuries during midlife. And this does NOT mean women in perimenopause should exercise less. We do need to be strategic about how much we load it and the recovery. Progressive resistance training. Adequate recovery. Enough protein and overall nutrition. Bone-loading exercise. And avoiding huge, sudden spikes in training volume. We can’t drive it like we stole it. PMID: 42332940

I’m not trying to be a pain, but we have to be intentional now. 1.) mobility 2.) resistance training This is really, really important. We want to keep the range of motion of your joints. We want to keep your joints supple. We want to keep our bones, muscles and tendons strong, but we can only do that by loading them. That means strength training, also known as resistance training. Everyone needs this, we all need exercise. Our quality of life truly depends on it.

16 years ago I graduated with my doctorate from the University of St. Augustine. I’m so glad I did, because it truly helped me become the women’s health physical therapist that I am.

I also did my manual therapy certification and was lucky enough to learn from Stanley Paris, Catherine Patla and Rob Stanbourough.

Rob was my mentor and truly pushed me, I’m forever grateful for his meticulous ways. I’m grateful for his love of science and soft tissue. These skills made me who I am. Understanding connective tissue gave me the confidence to listen to women and understand what was happening to what tissues. I honed my palpation skills, my manual skills and biomechanics. I loved almost every minute of it, except for my MTC exam where I told Stanley Paris he was wrong. We then became Facebook friends and went to dinner. 😭🤣

(he was wrong, he wasn’t listening)

I worked full time as a physical therapist, because I already had my master’s in physical therapy. So I worked all day and studied at night. I was exhausted. I remember falling asleep on my notes, eating Starburst Jelly Beans and gaining 20 pounds from said jelly beans.

I became so tired that I wanted to quit. So one day I did quit. I told myself I was done. I had the best day. It was so nice to not think. The next day: I resume life and carried on.

I was tired. I just needed rest.

(Does this sound familiar ladies? We need more rest.)

So be brave. Do the hard things. Jump in. Do it scared. It could change your life, I know I’m better for all the hard times. I’m better for the challenges, but most of all, it made me exactly who I am today.

A woman who can read, can learn, can change and change the world.

Glut Med Strengthening Exercises These glut med exercises are ranked from an EMG study. That doesn’t mean one is better than the other, we need to also take into account your pain and current strength levels. We want to challenge you and your muscle, where you currently are. So this is why, I still start women with non-weight bearing exercises like the straight leg raise or banded clam shell. I don’t meet women when they feel good, I meet women when they are broken. I meet them where they are and then, we test their strength. We will try different exercises and see which once challenges them. I have worked up to doing a side plank with a hip abduction with a 2 pound cuff weight. I didn’t start there, I eventually got there over time. That’s what will happen with you too! Except…there is ONE thing. If you have a full thickness glut med tear, there is no neural drive and you will not strengthen it. This is why it’s important to have someone help you with your rehab progression. What exercises did I miss?

My simple full body home workout using adjustable 25 pound weights This pair of adjustable weights by lifepro uses a simple toggle to go between the different weights. Very easy to use, doesn’t take up a lot of space.

You will never guess the number one reason you’re failing with exercise. It’s not lack of time. It’s not lack of equipment. It’s being overwhelmed. It feels too much. It feels too big, so you never start. Because you don’t need fancy things or fancy equipment. You don’t need a lot of time either. You need a simple beginning routine. A simple plan you really can do. Something you can truly do and you need the win. You need a bit of success. I started with walking 1.25 miles. I remember how hard it felt and how embarrassed I felt at the difficulty of it. I used to be somewhat athletic, now this? Ugh. You know everyone starts the same way, at the very beginning. So don’t feel bad about starting at the beginning. I suggest bicep curls and squats if you don’t know where to start. You can hold an item and curl it. You can do squats with your body weight. You can do a 5 minute walk after dinner. Or walk on your break at work. Squats after lunch. Tiny changes. Tiny programs are doable programs.

I made this in 2024. I was so broken. I changed my entire exercise routine after this. I added in mobility daily for 10 minutes. I added resistance training 3 times a week. I started walking 1.25-2 miles a day. I started tracking my fiber and protein. I started saunas and cold plunges. I started to change everything, week by week. Here I’m 205, 4/10 pain, exhausted and just done. I’m currently 159, function well and feel good most of the time. I share this with you because I want you to know, that I get it. I literally get it. I have crawled my way out of the hole, week by week. I’m not perfect now, but I feel a heck of a lot better now than I did here in this video. Love you bye
