At the recent Private Care Association National conference there was a recurring question from attendees. “Is MyHealthForm AHCA approved?”

Our response, “It is designed to be AHCA compliant.” AHCA does not approve or disapprove outside products.

When asked directly at this very conference AHCA representatives acknowledged and confirmed that if MyHealthForm follows statutes then it is compliant. We then went into the statute and how it is observed.

End result, while not AHCA approved, it has been acknowledged on multiple occasions, by multiple AHCA representatives, to be AHCA compliant.

MyHealthForm.com started because of a real problem identified by Abby Services In-Home Care , a nurse registry in Fort Myers, Florida.

Caregivers are required to provide documentation stating they are “free from communicable disease,” but the process was often expensive, inconsistent, and frustrating for everyone involved.

Many physicians were understandably uncomfortable signing broad blanket statements, and nurse registries were left trying to navigate how involved they should even be in the process.

After researching the actual statute, reviewing established communicable disease screening tools already used by organizations like the Wisconsin Department of Health and Mount Sinai Hospital, and attempting to seek clarification from AHCA, we realized there had to be a better solution.

That ultimately led to MyHealthForm.com, a simple digital communicable disease screening platform designed specifically around the Florida nurse registry model.

What started as a real operational frustration turned into a tool now helping caregivers and registries across Florida complete the process faster, easier, and more affordably.
