The Main Street Pharmacy Access Act (H.R. 3164), formerly the ECAPS Act, just passed the House Ways & Means Committee and now heads to the House floor.

What it does: Adds pharmacists to the Social Security Act as Medicare providers and opens Medicare Part B reimbursement for pharmacist-provided test-to-treat services (flu, COVID-19, strep, RSV) in states whose scope of practice already allows it.

Why it matters: Nearly 9 in 10 Americans live within 5 miles of a pharmacy. For seniors in rural and underserved communities, the pharmacist is often the most accessible clinician, and now Medicare can finally pay for that care.

Where Provider CSAO comes in: The infrastructure to bill medical benefits doesn't build itself on enactment day. Our network pharmacies are already credentialed, contracted, and billing commercial medical payers today. When H.R. 3164 is signed, we're ready on day one.

Pharmacists are providers. The reimbursement is catching up.

One of the most common questions pharmacists ask us is: What can I do in my state?

Finding that answer usually means digging through state legislation databases.

We think that information should be easier to access.

Our new public tracker lets users search by state and view passed and in-progress legislation related to pharmacist clinical opportunity, including scope of practice, reimbursement-related items, and even some select PBM reform. We also included direct legislation links, plus a section for ECAPS and other federal movements, so that you can read for yourself exactly what the law says.

This is our small step toward more transparency in pharmacy. We’re evolving—just like clinical pharmacy is.

Kentucky lawmakers have taken a big step forward with Senate Bill 38, expanding Medicaid reimbursement for pharmacist provided services like flu and strep testing. By allowing care to happen at community pharmacies, where nearly 90% of Kentuckians live within five miles, this legislation helps lower costs, reduce ER visits, and improve access, especially in rural areas. This is about letting lower cost providers care for patients where they already are.
