After a hospital or ER visit, don't assume your medication routine is exactly the same as it was before.
Your discharge instructions may include:
π A brand-new medication
π A different dose or schedule
π A medication you should stop taking

π Instructions to discuss certain medications with your provider
One of the most important things you can do after discharge is make sure your medication list is **accurate and up to date.**
Before your follow-up appointment:
βοΈ Gather ALL medications you currently takeβincluding over-the-counter medications and supplements
βοΈ Compare them with your discharge medication list
βοΈ Write down anything that looks different or confusing
βοΈ Bring your medication bottles or an updated list to your appointment
βοΈ Ask your pharmacist or healthcare provider about anything you don't understand
βοΈ Never start, stop, or change the dose of a medication without guidance from an appropriate healthcare professional
