A counterintuitive finding from ENDO 2026: when researchers tracked daily step counts in adults who started semaglutide or tirzepatide, average steps fell from 5,047 to 4,487 per day -- a drop of about 560 steps. Moderate-to-vigorous activity fell from 27.9 to 22.2 minutes a day.

The assumption was that losing weight on GLP-1s makes movement easier. The data says the opposite.

This was an observational wearable cohort, before and after starting therapy. It cannot prove the drug caused the drop -- nausea, fatigue, or simply eating less at home could contribute. But the direction and size of the signal are concerning.

Why it matters: GLP-1s reduce fat mass but also lean mass. Losing muscle while becoming more sedentary is a recipe for sarcopenia, insulin resistance rebound, and weight regain after stopping. The fix is not to abandon the drug -- it is to pair it with resistance training and a daily step floor.
