An interesting reading. Radiesse has always given me consistent reliable results and it has always been my preferred recommendation for my patients.

Main claim:
Both Radiesse (hyperdilute) and Sculptra stimulate collagen—but they behave very differently at the level of the extracellular matrix (ECM).

Key points:

* Radiesse (hyperdilute):
* Acts more like a scaffold
* Promotes organized collagen + elastin formation
* Leads to ECM that more closely resembles younger, healthier skin
* Lower inflammatory response (more controlled remodeling)
* Sculptra:
* Works via a stronger inflammatory stimulus
* Still builds collagen, but in a less organized way
* Higher risk of nodules (the post references ~17% in its FDA trial context)
* More “inflammatory-driven” collagen production rather than structural regeneration

Big idea:
Not all “collagen stimulators” are equal—
one is framed as regenerative (structured ECM rebuilding), the other as inflammatory (volume via collagen response).

Bottom line:
The argument is that Radiesse better recreates youthful skin architecture, while Sculptra relies more on inflammation-driven collagen with higher variability and risk.

Something to keep in mind—while these differences exist, your results ultimately depend on the injector’s technique, experience, and treatment plan.

Your Own Natural Filler… But Better

Facial fat grafting is one of my favorite in-clinic treatments because it does more than add volume — it restores, heals, and rejuvenates from within.

Unlike traditional fillers, fat comes from your own body, making it incredibly natural and biocompatible. But the real magic? The regenerative cells within the fat that improve skin quality, texture, and overall radiance over time.

At Hawai’i Facial Plastic Surgery, we offer both structural fat grafting (to restore volume and shape) and nano fat (to enhance skin quality, fine lines, and delicate areas like under eyes).

Why patients love it:
• Natural, soft, and harmonious results
• Improves skin quality — not just volume
• Longer-lasting than traditional fillers
• Can be done in-office with minimal downtime

This is for patients who want subtle, elegant rejuvenation — not overfilled or artificial.

The result isn’t just looking fuller… it’s looking healthier.

If you’ve been curious whether you’re a good candidate, we’re happy to guide you.

Before → After | Deep Plane Face & Neck Lift by Dr. Tan

A beautifully refreshed jawline, softened jowls, improved marionette lines, and restoration of a youthful V-shaped lower face.

The key to this result? The deep plane technique.

Unlike traditional lifts that only tighten the skin, a deep plane facelift releases and repositions the deeper facial structures (SMAS and ligaments). This allows for a more natural, longer-lasting lift—without that tight or pulled appearance.

The result:
✔️ Smooth, defined jawline
✔️ Natural lift of the mid and lower face
✔️ Harmony, not overdone

The right time isn’t about age—it’s about recognizing when subtle changes can make a meaningful difference.

One of the most common questions after rhinoplasty is: “Will my nose look natural?”

The short answer—yes, when it’s done thoughtfully.

A successful rhinoplasty isn’t about creating a “perfect” nose. It’s about creating your nose—one that fits harmoniously with your unique facial features. The goal is balance, not attention. In fact, the best results are often the ones no one notices.

Right after surgery, swelling can temporarily make the nose look different than expected. This is completely normal. As healing progresses over weeks to months, the nose refines, softens, and settles into a more natural appearance.

A natural-looking result depends on:
• Respecting your facial proportions
• Preserving structural integrity
• Avoiding overcorrection
• Tailoring the procedure to you, not trends

Remember: rhinoplasty is as much an art as it is a science. Choosing an experienced surgeon who prioritizes subtle, individualized results makes all the difference.

If you’re considering rhinoplasty, patience and realistic expectations are key—your final result is a gradual transformation, not an overnight change.

Why do lips change as we age? Let’s talk about it…

Ever notice your lips looking a little thinner or less defined than they used to? You’re not imagining it. As we age:

✨ We naturally lose collagen
✨ The lip border (that crisp outline) softens
✨ The upper lip can lengthen
✨ Fine lines can form around the mouth

The good news? Thoughtful, natural lip filler can restore what time has taken away — without looking overdone.

A skilled injector doesn’t just “add volume.” We can:
💉 Restore the youthful lip shape
💉 Define the vermilion border
💉 Support the corners of the mouth
💉 Smooth fine lines around the lips
💉 Create balance with the rest of the face

The goal is refreshed, hydrated, naturally beautiful lips — not “filler lips.”

Think: your lips, just a little earlier in time.

At Hawaiʻi Facial Plastic Surgery, we specialize in subtle, customized lip rejuvenation that enhances your natural beauty.

Nose inspiration ➡️ surgical artistry ➡️ your unique result

Every rhinoplasty starts with a conversation. We look at inspiration photos together, discuss facial balance, and design a nose that fits your features—not someone else’s. The goal is refinement, harmony, and a result that looks naturally yours.

Education is the first step in great results. I love guiding my patients through the process so they understand what’s possible and what creates a beautiful, balanced outcome.

If you’ve ever saved a nose inspiration photo… this is how we turn that vision into reality.

Planning a facelift? Let’s talk timing with injectables & energy treatments — based on what we know (and don’t know).

Facelift surgery is a foundational procedure for facial rejuvenation — but it doesn’t fully correct volume loss, skin quality changes, or dynamic wrinkles. That’s why many patients use:

💉 Neurotoxin
💉 Dermal filler
🧬 Biostimulators (Sculptra, Radiesse)
⚡ Energy-based devices (RF, ultrasound, lasers)

…before or after surgery.

But how does this affect surgical outcomes?

A small retrospective review published in Journal of Cosmetic Dermatology looked at 20 facelift patients with prior minimally invasive treatments:
• 100% had neurotoxin before surgery
• 90% had hyaluronic acid filler
• 55% had biostimulators

• Many resumed injectables within the first year after surgery

In this small cohort, prior injectable or energy-based treatment was not associated with increased surgical complications.

However — and this is important — the study was small, retrospective, and not designed to definitively assess safety or risk.

🔎 From a surgical planning standpoint, recently placed filler, collagen stimulation, or energy-induced tightening can temporarily alter anatomy and tissue behavior. That can make it harder to assess true laxity, volume loss, and vector needs.

For patients considering surgery within the next year, I generally recommend:
⏳ Avoid filler several months before surgery
⏳ Avoid biostimulators ideally 12 months prior
⏳ Avoid energy-based tightening treatments several months prior

Surgery should be performed on stable, natural anatomy whenever possible. Sequencing matters.

Larger prospective studies are still needed to define best practices — but thoughtful timing remains key to achieving the most predictable, natural outcome.

If surgery may be in your future, let’s plan strategically 🤍
