Little did we know hiring Kara would be the most pivotal point in company history. She has shown dedication we have never seen before. She has stood before judges to advocate for our clients, she has arranged 3rd party providers organized like ducks in a row to accomplish client/family goals, and she even extended grace to a client's family member who gave her a good heaping of trouble by picking them up from the airport. Your heart, your burning desire to serve others, and your passion for your role inspire the WHOLE team. Thank you for all you do, kara. When a caregiver had an emergency and Saving Grace's administrator, Kara, had to help someone else; Josh stepped in to help the team. What happens when a patient is medically ready to leave the hospital… but no one is willing or able to step in?

In this scenario, our home care administrator is trying to advocate for a vulnerable patient. There are concerns that may warrant an Adult Protective Services (APS) report, but after being told that hospital policy limits what can be shared, they're left trying to figure out the next step.

This isn't about placing blame on healthcare professionals. Many case managers are doing everything they can within the policies they're required to follow.
It's about highlighting a reality that happens more often than people realize: vulnerable adults can fall into the gaps between systems.
