RHINOPLASTY

This is my beautiful 22-year-old patient local to Houston who came to me wanting a more refined, feminine nose while preserving the natural character of her features.

At 4 months post-op, the change is already striking from every angle. Her main concerns were a prominent dorsal hump and a bulbous, under-projected tip. We carefully reduced the dorsum to create a smooth, elegant profile and refined the tip to give her better definition and a softer light reflex, without over-narrowing or losing femininity.

From the front, you can see a more balanced nasal width and improved harmony with her cheeks, eyes, and lips. On profile, the dorsal line is now clean and continuous, with a subtle supratip break and a tip that sits in beautiful proportion to her chin and upper lip.

She’s still early in the healing journey, with some expected residual tip swelling that will continue to refine over the coming months. I can’t wait to see her final result as everything settles.

LOWER EYELID BLEPHAROPLASTY

This is my 36-year-old male patient, local to Houston here 3 months after lower eyelid blepharoplasty.

Some people call it the trifecta lower lid bleph. The simplest way to think about it, though: this is restoration of the eyelid at several levels at once.

The tired under-eye is never one thing. It’s usually three.

(1) There’s the deep fat that’s herniated forward and casts a shadow.

(2) There’s the tethering — the tear trough ligament and the orbital retaining ligament hold the lid-cheek junction down, which is part of what carves that hollow in the first place.

(3) And there’s the skin itself, with its own texture and crepiness that fat work alone won’t touch.

So we address all of it in one go. The fat gets repositioned or excised. Those ligaments get gently released, so the hollow softens instead of staying pinned. The lost volume gets restored with fat grafting. And the skin gets resurfaced, usually with a peel or CO2 laser, so it actually matches the smoother contour underneath.

Fix one and ignore the others and the eye still looks tired.

Handle them together and it just looks like you slept well.

Rhinoplasty + Accutane. 4 months post-op.

This was one of the more challenging cases I’ve taken on. Thick sebaceous skin is the hardest variable in nasal surgery — the cartilage framework can be built exactly to plan, and the real battle becomes getting the soft tissue envelope to redrape over it. Heavy, opaque skin holds edema and resists contraction for a full year, sometimes longer.

A tailored post-op Accutane protocol changes the timeline. Sebaceous glands shrink, the dermis thins, and the envelope finally lays down over the new structure.

Four months, not twelve. And it only gets better from here.

FACELIFT

What it does. What it doesn’t. And the part most patients miss.

A facelift lifts and repositions tissue that has dropped with time. The jawline sharpens. The neck tightens. The underlying structure of the face is restored.

Here’s the part most people get wrong: a well-executed facelift can also make the face look fuller. Nothing has been added. Your own tissue is back where it used to sit. That’s why a great result reads as rested, not pulled.

What a facelift will not do is change your skin.
Fine lines, texture, sun damage, crepey skin. None of it improves with surgery alone. Skin quality is a separate problem with separate solutions: lasers, peels, and the right skincare.

The best outcomes come from knowing what each tool is actually built for, and using them in the right order.

WHY I DON’T MORPH NOSES

Every week a patient asks me to show them what their nose will look like after surgery.

I get it. You want certainty before you commit.

But I don’t use morphing software — and I think it actually protects you.

A morph is a 2D edit on a flat photo. Your nose is a 3D structure that moves, ages, and lives on a real face. A computer doesn’t account for skin thickness, healing, swelling, or facial harmony. It just makes the nose smaller and calls it done.

Worse — once you’ve seen that image, you’re locked in. Even if your actual result is beautiful, it can feel “off” if it doesn’t match a picture a computer generated in 30 seconds.

What I actually want you to bring to your consultation: photos of noses you love and noses you hate. Both are equally useful. The dislikes tell me just as much about your aesthetic as the likes — sometimes more.

That conversation gets us closer to the real goal than any edited image ever could.
