That does not always mean the communication was unclear.

The decision may have been understood.
The roles may have been named.
The next step may have been visible.

Then the work moves across messages, functions, and follow ups.

When the same point keeps reopening, the issue may not be the message.

It may be the capacity required to hold clarity across repeated demand.

The visible response may be correct and still incomplete as the explanation.

A decision can be finished in the meeting and still come back two days later with the same people and no new information.

That is usually where the confusion starts.

The room already did the work.

The conclusion was already reached.

The decision still returns.

So more follow up gets added.

More time gets spent around something that should already be moving.
More attention gets pulled back into the same point again.

Nothing dramatic happens.

It just takes more effort to keep one decision from becoming an active conversation for a second time.

That is how the pattern keeps moving through a team without looking like failure.
