Sometimes an implant alone isn’t enough. That’s not a failure — it’s just the honest read on the anatomy.

One year out from a breast augmentation with a small donut mastopexy. She needed a bit of lift along with the implant, not just volume. Skip the lift on a case like this and the ni**le sits off-center and the breast tissue droops off the front of the implant — the shape never quite resolves.

Together, the two do what neither could alone: the implant gives volume, the donut lift recenters the ni**le and keeps the breast from sliding off it. One surgery, shape corrected.

One year in, healed and settled, and the shape holds exactly the way it should.

Fall is here, the weather is cooling down, and it’s one of the best times of year to start planning ahead.

If you’ve been thinking about doing something for your face or body, now is a great time to schedule a consultation at Trillium Plastic Surgery & Aesthetics.

Maybe it’s something you’ve been considering for years.

A facelift.
A neck lift.
Breast or body surgery.
A laser treatment.
Or maybe you simply know there’s something you’d like to improve, but you’re not sure what your options are.

That’s exactly what a consultation is for.

We can talk about what’s bothering you, what kind of result you’re hoping for, what actually makes sense for you — and sometimes, what doesn’t.

Fall and winter also give you time to plan around recovery, holidays, travel, and the year ahead instead of waiting until the last minute.

If you’ve been saying, “Maybe someday…”

Maybe this is the season to at least have the conversation.

Schedule your face or body consultation with myself or Jessica Vavra Walker, MD, FACS at Trillium Plastic Surgery & Aesthetics in Knoxville.

Working in plastic surgery for as long as I have has made me pretty opinionated about aesthetics.

There are a lot of treatments I like. There are also a lot of things I personally wouldn’t do to my own face.

Are there any of these do you disagree with?

And if you work in plastics or aesthetics, add yours below. What would you NEVER do to your own face?

One of the easiest ways to price your practice wrong?

Copying the person down the street.

You don’t know their overhead.
You don’t know what they pay for anesthesia, implants, staff, rent, or supplies.
And you definitely don’t know whether they’re actually profitable at that price.

Amanda and I go back through our pricing every year because costs change, volume changes, vendor pricing changes — and what worked before may not work now.

Your pricing should be based on your numbers, not somebody else’s.

Our full episode on pricing for profitability drops tomorrow on The Trillium Show. Subscribe on YouTube or wherever you listen to podcasts so you don’t miss it!

It’s officially laser season⚡️

As the weather cools down and we spend less time in the sun, fall and winter are some of the best times to start treatments that target things like:

• Sun damage
• Pigmentation
• Uneven skin tone
• Fine lines
• Texture
• Acne scarring

Why now?

Many laser treatments require you to be more careful with sun exposure before and after treatment, which makes the cooler months an ideal time to get started.

And depending on your skin and your goals, you may need more than one treatment — so starting now gives your skin time to improve over the next several months.

The biggest thing I tell patients: not every laser is right for every problem.

The goal isn’t to pick the newest or most aggressive device. It’s to figure out what you actually want to improve and choose the right treatment for your skin.

If you’ve been thinking about doing something for sun damage, texture, pigmentation, or overall skin quality, now is a great time to have that conversation.

This was one of the parts of my conversation with Roman Bystrianyk, co-author of Dissolving Illusions, that made me stop and think.

We spent a lot of time looking at historical infectious-disease mortality and asking a question I was never really taught to ask in medical school:

What was already changing before vaccines arrived?

The historical data Roman walked through showed dramatic declines in deaths from diseases like measles, whooping cough, scarlet fever, and tuberculosis during the same era that clean water, sanitation, nutrition, housing, and overall living conditions were improving.

That doesn’t mean vaccines had no effect.

It does mean the history is more complicated than simply saying, “Vaccines came along and infectious disease disappeared.”

And I think we should be able to talk about that distinction.

Look at the data.
Look at the timeline.
Ask what changed first.

Then decide what you think.

Do vaccines get too much credit, too little credit, or the right amount?

Keep it civil. I genuinely want to hear both sides.

Not every breast story starts with “what went wrong.”

Most of what we post is revision work — old implants, sagging, lifts. Fixing what didn’t hold up over time. But that’s not the whole practice.

This patient came in with nothing to correct. Just a clear goal: proportion that matched her frame, and a result that still looked like her.

A primary augmentation, planned around her anatomy from the start — implant size and placement chosen for how she actually moves and dresses, not a one-size template.

No implant lasts forever, and that’s not a flaw — it’s just biology. But a well-planned primary augmentation gives her a great result now, and a much easier revision down the road if she ever needs one.

A practice can be growing on paper while quietly becoming less profitable.

That’s why I think you have to step back at least once a year and look at the real numbers again.

Your overhead. Your time. Your staffing. Your supplies. Shipping. Cost of living.

Those expenses creep up gradually, which makes them easy to ignore — but eventually they start eating into your margins.

And one of the biggest mistakes you can make as a business owner is assuming the numbers that worked a year or two ago still work today.

Revenue can go up while profitability goes down. Know your numbers.

I’m releasing a podcast episode all about profitability next Thursday, 10/8. Be sure to subscribe to The Trillium Show on YouTube or wherever you listen to podcasts so you can learn how to build a more profitable, sustainable practice — and avoid the expenses that quietly eat away at your bottom line.

One year out. Dorsal hump removed, tip refined — that’s it.

She’d wanted this since she was a teenager. Not a new face. Just her nose, without the bump she’d been self-conscious about for over a decade.

That’s what I hear most: minimal change, maximum “still me.” No one wants to look operated on. They want to look like themselves — just the one detail fixed.

One year post-op, healed and settled, and she says it’s everything she wanted.
