Ohana Care Connections

Ohana Care Connections Ohana Care Connections supports seniors and their families in navigating assisted living and memory care communities.

We simplify the process of finding the ideal home that meets each individual's unique needs. Ohana Care Connections was founded by Taylor Mauhili, President and CEO. Being born and raised in Hawaii, you learn from a young age how important family is. In Hawaiian, ohana means family—a word and value I grew up with. Being there for both the good times and the hard times makes you stronger as an enti

re ohana. I began volunteering at a local independent living, assisted living, and memory care community from a young age, as my mother worked there for many years. I found joy and fulfillment in spending time with each resident—getting to know them, participating in activities, going on outings, and watching them live each day to the fullest. As I got older, my passion for working with seniors only deepened. I wanted to ensure they received the care they needed to enjoy the highest quality of life possible. I worked for many years in hospice, where I truly felt at home. I was able to advocate for seniors, connect them with vital resources, and support them through their journeys with terminal illnesses. Later, I transitioned into the world of assisted living and became a Lifestyle Director for a secured memory care community specializing in dementia and Alzheimer’s. I loved interacting with the residents, ensuring their needs were met, and helping fill their days with purpose and meaningful engagement. Watching their faces light up as we went into the community to raise awareness about dementia and Alzheimer’s, took them out for lunches, hosted live concerts, and celebrated holidays together, made me realize this is truly my calling in life. I’ve also had the opportunity to work for organizations that help clients and families navigate assisted living, memory care, and in-home caregiving options. I’ve guided many clients through the state long-term care application process and advocated for their approval for state-funded assistance. This work helped secure in-home care or assisted living services for individuals who could not afford care on their own and had high levels of need their families could no longer manage. Living in a rural city, I’ve seen firsthand the gaps in medical services, the shortage of resources, and the financial challenges many families face when it comes to care. I want to be part of the change—helping seniors improve their quality of life by connecting them with the resources and support they need to truly thrive.

Meet Another Member of Our Ohana🌺💛

Today, we're honored to introduce another member of our Ohana, Lori.

Lori and Taylor from Ohana Care Connections first met when a close family friend of Lori's was hospitalized at Havasu Regional Medical Center and needed a safe discharge plan. At the time, he required skilled nursing care, and unfortunately, there were no local skilled nursing options available. We discussed assisted living, but his goal was to regain strength before making that transition.

Together, Havasu Regional Medical Center, Lori, and her friend worked diligently to coordinate a plan, and he was ultimately transferred to a skilled nursing facility out of our county. Although he had left the hospital, our journey together was just beginning.

Taylor and Lori remained in close contact throughout his stay, helping advocate for him during the ALTCS application process and discussing plans for his future care. We knew that when he was ready, we would be there to help him find a safe and appropriate forever home. Countless conversations were spent discussing his needs, his goals, and how we could help ensure he had every opportunity to succeed.

Unfortunately, life had other plans.

Late one evening, Lori received the heartbreaking news that her friend had unexpectedly passed away.

The loss was devastating, and during that difficult time, Lori and Taylor continued to support one another as they discussed next steps and reflected on his journey.

Not long afterward, Lori and Taylor finally had the opportunity to meet in person over lunch.

What was supposed to be a simple lunch turned into an incredible afternoon of conversation, connection, and friendship. Taylor had the privilege of learning more about Lori's life and the countless ways she has cared for others throughout the years. One of the most inspiring things Taylor learned was that Lori had helped care for 11 other seniors simply out of the kindness of her heart.

That level of compassion is rare, and it perfectly embodies what Ohana means.

At the end of their lunch, Lori surprised Taylor with a small gnome as a gift.

It may seem like a simple gesture, but it meant the world.

Today, that little gnome sits on Taylor's desk as a daily reminder of the relationships built through this work and the impact that can be made when people come together to support one another during life's most difficult moments.

This is why Ohana Care Connections exists.

We are not just a senior placement agency.

We are Ohana.

When families call us, they aren't getting a referral list and being sent on their way. Taylor takes the time to truly get to know each senior and their family, understanding their medical, emotional, psychological, and financial needs to ensure the right assisted living or memory care community is selected.

Whether you're in a hospital, skilled nursing facility, rehabilitation center, your home, or even sitting across from Taylor at a coffee shop, she will meet you where you are and walk alongside you through the process.

Because finding the right home is about more than placement.
It's about creating a safe, stable foundation and ensuring every senior feels valued, supported, and cared for.

And long after the move is complete, you'll always remain a part of our Ohana.

It is our mission to make you feel like part of our Ohana. Together, no one is left behind or forgotten🌺💛

Ohana Care Connections would like to introduce another incredible member of our Ohana, Carlos🌺💛

With Carlos' permission, we would like to share his story.

Sometimes the calls we receive at Ohana Care Connections are not just about finding an assisted living—they're about helping someone survive long enough to get there.

Recently, Taylor Mauhili, President & CEO of Ohana Care Connections, received an emergency call from a local agency regarding a senior in desperate need of help.

After learning more about his situation, Taylor immediately reached out and connected with Carlos.

Carlos was living in a motel with very little food, limited clothing, and few basic necessities. He needed hygiene products, support, advocacy, and most importantly, a safe place to call home.
Without hesitation, Taylor began gathering resources.

She went through clothing donations that had been generously provided to Ohana Care Connections, packed a suitcase full of clothes, gathered hygiene products, purchased food to help Carlos get through until his move, and even bought him a new pair of tennis shoes after learning how badly he needed them.
Once everything was ready, Taylor drove out to Carlos's motel room to deliver the supplies personally.

The moment Carlos received the food, clothing, and shoes, he became emotional. He was overwhelmed with gratitude and moved to tears knowing that someone cared enough to help. This 3 hour long first in-person meeting has made such a huge impact in Taylor and Carlos' life.

Carlos shared stories about his life, his health, the challenges he had faced, and the circumstances that had led him to where he was. Despite everything he had endured, he remained kind, grateful, and full of humor.

After putting on his new shoes, he smiled and said:
"These are the most comfortable shoes I've had in a long time. I can't tell you how long it's been since I've had tennis shoes."
Taylor jokingly nicknamed him "The Flash," although Carlos quickly replied that he might be the slowest-moving Flash around😊

Even in the midst of difficult circumstances, he never lost his sense of humor.

This journey could not have been accomplished alone.
A special thank you to Living Waters Hospice, who immediately stepped in to provide exceptional medical care while Carlos awaited placement. Their team also went out of their way to ensure he had food and additional support during this critical time.

The day finally arrived when Carlos was ready to move into his assisted living community.

Taylor helped pack his belongings, load the vehicle, and get him safely on his way to his new home.

Like many seniors facing a major life transition, Carlos was nervous. But he was also excited knowing that he would finally have the care, stability, and support he deserved.

These are the types of calls Ohana Care Connections receives every day.

Calls about seniors who are scared, vulnerable, alone, and in need of someone to advocate for them.

Someone to help them navigate the healthcare system.

Someone to help them find hope.

Someone to remind them that they matter.

Today, Carlos is safe, cared for, and surrounded by support.

And just like every senior and family we have the privilege of serving, Carlos will forever be a part of our Ohana.

We are incredibly grateful that we were able to walk alongside him and help him find his forever home.

It is our mission to make you feel like part of our Ohana. Together, no one is left behind or forgotten🌺💛

Meet Another Member of Our Ohana💛

The photo you're seeing today is more than just a picture. It's a reminder of why Ohana Care Connections exists.

We first met Teresa during one of the most difficult times of her life. Her father was in a skilled nursing facility in California, and she was desperately trying to figure out a safe discharge plan. She didn't know where to begin, what resources were available, or how she was going to bring her father home to Arizona.

As fate would have it, President & CEO Taylor Mauhili happened to meet Teresa while visiting local assisted living communities. After being introduced, Taylor shared information about ALTCS, assisted living pricing, and how Ohana Care Connections could help guide her through the process.

From that moment on, we were in it together.

For the next week, Teresa and Taylor spoke on the phone or met in person nearly every day. Together, they explored options, coordinated care, communicated with the skilled nursing facility, gathered physician orders, and worked through every detail needed to ensure a safe transition.

When an assisted living community was finally selected, Taylor and Teresa sat together until 9:30 PM completing residency paperwork, ensuring physician orders were received, transportation was arranged, and every detail was in place.

Then the day finally arrived.

Teresa was able to bring her father back home to Lake Havasu City. He was safely settled into his new assisted living community, surrounded by care and support.

Sadly, just five days later, he passed away.

While Teresa's time with her father was shorter than anyone hoped, she was incredibly grateful for the care he received from the assisted living community, the transportation team, hospice, and Ohana Care Connections.

One thing she said has stayed with us ever since:
"You were sent to me by my mom. She sent me another Taylor out here in Arizona to help me find the care I needed for my dad. I am so grateful to have had your help through this journey."

Moments like these are exactly why Ohana Care Connections exists💛

Our services are completely FREE to seniors and families because we believe no one should have to navigate the senior care journey alone. We don't just help families find an assisted living or memory care community—we walk beside them every step of the way.

Many of you may remember the long trip Taylor recently made to Illinois to help another client and their family relocate. Well, before that trip, Teresa called out of the blue with a few questions. During the conversation, Taylor mentioned she would be traveling through Teresa's area on the way back from Illinois.
Teresa immediately said she would love to see us if we had time.

After coordinating with transportation, we made it happen.

A few weeks later, and after many miles traveled, Taylor from Ohana Care Connections, Avery from Horizon Health Transport (who had transported Teresa's father to his assisted living community in Lake Havasu), and Teresa were finally reunited for brunch.

It was wonderful to see Teresa again, experience a small part of the country near her hometown, and reconnect with someone who had become part of our Ohana.

At Ohana Care Connections, our relationship with families doesn't end once a move-in is complete. Whether it's days, months, or years later, we're still here.

Because our mission has never been simply helping people move into assisted living or memory care.

Our mission is to make you feel like part of our Ohana. Together, no one is left behind or forgotten🌺💛

If you or a loved one is wanting to explore assisted living or memory care communities, Ohana Care Connections is available 24 hours, 7 days a week. Call 928-224-9029 to begin your journey!💛

Ohana Care Connection had the absolute privilege of throwing a carnival for the wonderful residents at Haven Health Lake Havasu🎪🍿❤️

First, we want to say a massive THANK YOU to Haven Health for welcoming us with open arms. Seeing the residents’ faces light up, hearing their laughter, and watching them win prizes and enjoy fresh-made popcorn was pure magic. There is truly nothing better than sharing those beautiful, joyful moments together. Their activities director, Misty was so amazing to work with and made it so easy to get all of this planned together.

An afternoon like this thrives on community, and we were blessed to have some truly amazing friends help us pull it off. We want to send a huge, warm thank you to the incredible organizations who poured their love into yesterday's festivities:
-Maggie's Hospice🕊️ Their fish bowl game was so festive and fun!
-Maggie's Dream Team 💖 Ring toss was such a huge hit, the residents kept going back for more!
-Havasu Regional Home Health 🩺 Balloon pop was a huge hit! Hearing the ballon pops really made it feel like we were at the carnival!
-Compassus Hospice🕊️ The generous raffle baskets they made to give out to two lucky residents were phenomenal!
-Devin from Canyon Home Care and Hospice– who jumped right in to volunteer at the Ohana Care Connection’s duck pond carnival game and brought so much fun energy!🐥

To everyone who helped, cheered, and played: thank you for reminding us what "Ohana" is all about. We have such a compassionate community here in Lake Havasu, and we are so grateful to walk this path with all of you💛

Take a look at some of the sweet smiles from yesterday! 📸
