There is so much conflicting advice out there about appropriate infant weight gain. As lactation consultants, even what we were taught during our training conflicts with both current and past research on appropriate weight gain. We were always taught at least 0.5 to 1 oz a day is appropriate but when you plot a baby who is growing at this rate on any growth chart, the numbers don’t add up. So we decided to take a deeper dive into the research and apply this in clinical practice. Not all milk supply challenges are fixable with more pumping.

We’ve traditionally talked about supply in terms of hormones and stimulation. And those things absolutely matter. But newer research is showing there’s another layer that starts way earlier, during pregnancy.

Your body is building the actual milk-producing cells months before your baby is born. Some of those cells become “high output” cells that are capable of making larger volumes of milk.

And here’s the part that can be hard to hear but important to understand… some people simply develop fewer of those high capacity cells.

Which means no matter how perfectly you latch, how often you pump, or how committed you are there can be a ceiling.

This doesn’t mean support isn’t important. It doesn’t mean we don’t optimize everything we can.

It means if things aren’t increasing the way we expect, it’s not always because something is being done wrong.

There is real biology behind this.

If you’re struggling with supply, you deserve support that looks at the full picture. Mechanics, hormones, and your body’s underlying capacity.
