🤍 Maternity Leave Update 🤍

Baby time is almost here! I am tentatively planning to be out of the clinic September 8 through November 16.

While I’m away, Jimmie Barnwell, PA-C, a provider I’ve worked closely with in the past, and Jake Cail, PA-C, owner of Headwaters Direct Primary Care, will be taking great care of you!

🩺 Clinic coverage:
We plan to have a provider in the clinic Monday–Wednesday, with Jake available as needed on Fridays. Schedules may occasionally change due to other work commitments, so please reach out early when possible so we can accommodate your medical needs.

📱 IMPORTANT COMMUNICATION UPDATE:
During my maternity leave, please CALL OR TEXT 406-284-2021 for ALL clinic communication.

Please DO NOT text 406-612-4263 while I am away. Messages sent to that number will only come directly to me and may result in a delayed response.

When texting 406-284-2021, please include your name + date of birth so our covering providers can easily identify you.

🥇 Physicals cost $25 per athlete
🥈 Appointments required
🥉 Forms available at the clinic

🗓️To schedule, please call 406-284-2021 or text 406-612-4263. Please schedule early as our appointments fill up quickly in August.

If your child had a physical last year, please read the form as the frequency of sports physicals changed. If the physical was completed after May 1st, it is valid for two years. However, please reference your school’s specific requirements.
