In 2004, I decided to leave my broadcasting career behind and begin working toward a career in medicine. I was driven to medicine for a variety of reasons, including my own histories with cancer. I found those experiences as a patient to be utterly invaluable as I learned the art and science of doctoring, and I know my patients are glad that their doctor has that patientโ€™s perspective.

You have not changed a thing. You are hitting your protein, you are training, you are consistent. And the weight has stopped moving.
So you start doing the math. Eating a bit less. Adding cardio. Wondering what you are doing wrong.
You are not doing anything wrong.
Your brain noticed it was losing and decided to defend your weight. It dropped your core temperature. It slowed your heart rate. It quietly took your motivation, until all you want to do is lie down.
And it cannot tell the difference between a medication you chose and a famine you did not. It reacts to both the same way.
So a plateau does not mean the treatment failed. It does not mean you are bad at this. It means your body is doing exactly what it was built to do. It is evidence that you are normal.
What it does mean is that the plan needs adjusting by somebody who can see your numbers. A plateau is a clinical signal, not a character flaw, and it is one of the most common reasons people quit right before the part that works.

You cannot tell your body fat percentage by looking in the mirror. I could not either.
I have tracked my body composition casually for years. I prioritize protein. I lift. So when I finally ran the full scan on myself, I expected it to come back fine.
It came back at 32.68 percent. On the reference chart for women, 32 and above is the at risk category.
142 pounds. 96 pounds of lean mass. 46 pounds of fat. My waist is 30 inches, my hips are 38, and that ratio is 0.82, which is moderately risky for a range of metabolic problems.
Higher than I imagined, and higher than it looks on me.
Here is the part worth keeping. You cannot assess this by eye, not on yourself and not on anyone else. This happens to doctors too. We are not immune. The difference is not that I am immune. It is that I measured it.
If nobody has ever measured this on you properly, that is where to start. We run full body composition in the office and build the plan around your numbers instead of a chart average.

Nobody ever measured you.
You have been on this medication for months. Maybe longer.
No body composition. No protein target. Nobody mentioned strength training. You were handed a prescription and told to come back.
And now you are wondering whether you have already lost the window.
You have not.
The best time to do all of this was the day you started. The second best time is right now.
Yes, we will be working slightly uphill. Your metabolism has already adapted, and you have probably lost muscle you will need to rebuild. There are also screenings worth doing, because sometimes there is an underlying condition driving the whole thing that can be treated directly.
But understand this clearly. You are not a lost cause. You are not a failure. You did not do anything wrong. You were guided a certain way, and it just happens not to be the standard of practice.
So let's get you in line with the standard of practice.
That starts with one appointment. Body composition, a protein target set from your own lean mass, the labs that should have been run at the beginning, and a plan you can actually follow.

You did everything right.
You hit your protein. You did your training. You were consistent for weeks, and you have the receipts.
***
And then the scale stopped.
So you did what everyone does. You ate a little less. You added cardio. You started wondering what was wrong with you.
Here is what actually happened.
***
The part of your brain that manages energy noticed it was losing and treated it as a threat to your survival. So it got efficient. It dropped your core temperature. It slowed your heart rate. It quietly took your motivation until all you wanted to do was lie down. And it started wasting off muscle on purpose, because muscle is expensive to run.
That part of your brain cannot tell the difference between a plan you chose and a famine you did not.
So a plateau does not mean the treatment failed. It does not mean you are bad at this. It means your body is doing exactly what it was built to do.
It is not failure. It is evidence that you are normal.
But a plateau is a signal to look at the whole picture, not to cut harder. If nobody has checked your body composition, your protein target or your labs, that is exactly what a first appointment is for.

Hereโ€™s a fascinating overview of the psychology of food and part of why we have seen such an exponential drive to the obesity problem in the US in the last few decades.

It always goes back to one of my favorite sayings: โ€œthink about the equipment you have been issued.โ€ How has nature designed your brain and metabolism to surviveโ€”it has been hijacked by the modern food industry.

So in the face of this manipulation, we have to intentionally take back our food and help restore our metabolism to function as it was designed to do.

The Trump administrationโ€™s move to discontinue the subsidy is raising concerns among Medicare Part D enrollees about how much theyโ€™ll pay for coverage.

For the first time in your life, the noise stopped.
If you have lived with food noise, you know exactly what that means. Where can we eat. When can we eat. What can we eat. A running commentary that never gave you a minute's peace.
Then you started the medication and it went quiet.

People tell me it felt like being let out of something. They start thriving. They get results they had been chasing for years.
And then, a few months in, the hunger starts coming back. And they panic.

They tell me the medicine has stopped working, because they had come to believe that feeling nothing was the goal.
It is not.

These are tools. They are meant to help your appetite and your fullness, not switch appetite off entirely. You have to stay nutritionally replete or the weight loss will not hold.
Having no appetite at all is not the medication working. It is the thing I worry about.

If you are eating almost nothing and nobody has checked whether you are still getting what you need, that is worth an appointment. We look at what you are actually taking in, what your lean mass requires, and whether anything is being missed.

๐Ÿšจ A lab at Stanford just cured type 1 diabetes. No insulin. No immunosuppression. No daily anything.

The catch? It was in mice. But make no mistake, this is a real discovery. ๐Ÿงฌ

In late 2025, Stanford combined an islet cell transplant with a blood stem cell transplant and reset the immune system itself. The mice made their own insulin again. No rejection. No graft-versus-host disease. None of the brutal anti-rejection drugs that made every previous "cure" attempt not worth it.

Here is what I will tell you as an endocrinologist, and I do not say this lightly: with how fast this technology is moving, I think it is possible we see a cure for type 1 diabetes within my career. ๐Ÿ‘€

I am not selling you hope. I am telling you the science finally earned it. Watch the full breakdown, and I will keep you honest on what is real versus what is hype.

It happens to doctors too. Here is my story, part 3: the fight back. ๐Ÿฉบ

No crash diet. No punishment. ๐Ÿšซ I am shopping and cooking my own food and making intelligent swaps: mayo out, savory avocado spread in ๐Ÿฅ‘. More fiber, better fats, fewer calories, on the same turkey sandwich I love.

๐ŸŽฏ Target: 25% body fat by September 20. My numbers say it will not take years, and that is the whole point of knowing them.

This is the same process I use with my patients. Find out what you are made of, then decide. I am just doing it in public this time. ๐ŸŽฅ

๐Ÿ“บ Full plan is on my YouTube channel: search Dr. VanDyke and open the latest video.

It happens to doctors too. Here is my story, part 2: how it actually happened. ๐Ÿฉบ

In August 2024 I was in the best shape of my adult life. Best muscle mass, lowest fat mass. I remember looking in the mirror and feeling good about myself. ๐Ÿ’ช

One month later I herniated a disc in my lumbar spine. Surgery. And a recovery that took a lot longer than anyone warns you about. The lean mass never fully came back. The fat kept climbing. ๐Ÿ“ˆ

Here is the part I want you to hear: it was not willpower. It was life. ๐Ÿฉน

That is exactly why I will never hand you "just try harder" as a plan. Not to my patients, and not to myself. Bodies change because of injuries, surgeries, hormones, age, and a hundred things you did not choose. The answer is a real strategy, not a guilt trip.

So now I am rebuilding. Strength training four times a week ๐Ÿ‹๏ธโ€โ™€๏ธ, and every decision based on my own data, not guesswork.

The full story, from the injury to the comeback plan, is on my YouTube channel. Link in the first comment. ๐Ÿ‘‡
