No crash diet. No punishment. 🚫 I am shopping and cooking my own food and making intelligent swaps: mayo out, savory avocado spread in 🥑. More fiber, better fats, fewer calories, on the same turkey sandwich I love.

🎯 Target: 25% body fat by September 20. My numbers say it will not take years, and that is the whole point of knowing them.

In August 2024 I was in the best shape of my adult life. Best muscle mass, lowest fat mass. I remember looking in the mirror and feeling good about myself. 💪

One month later I herniated a disc in my lumbar spine. Surgery. And a recovery that took a lot longer than anyone warns you about. The lean mass never fully came back. The fat kept climbing. 📈

Here is the part I want you to hear: it was not willpower. It was life. 🩹

That is exactly why I will never hand you "just try harder" as a plan. Not to my patients, and not to myself. Bodies change because of injuries, surgeries, hormones, age, and a hundred things you did not choose. The answer is a real strategy, not a guilt trip.

I am an endocrinologist. Helping people manage their weight is literally my job. And for years I did everything I tell my patients to do: I prioritized protein, I lifted 🏋️‍♀️, I lived my normal maintenance lifestyle, and I figured that was enough.

Let me tell you about a drug that is not for sale yet, and probably still change how you think about weight. 🧬

Every GLP-1 you have heard of works one hormone, maybe two. Retatrutide works three at once. GLP-1, GIP, and the interesting one, glucagon, which is the receptor nobody was brave enough to build a weight loss drug around until now.

So what happens when you hit all three? In the phase 3 obesity trials, people lost up to 28.3% of their body weight, and around 30% in severe obesity. Read that again. Those are numbers we used to only see after bariatric surgery. From a shot.

Here is the honest part, and the reason I am not writing you a prescription today: it is still investigational. Not FDA approved. So this is a preview of where obesity medicine is going, not something you can fill this week.

But you do not have to wait for the future to get real results. The tools that exist right now, used properly and monitored properly, work. That is the plan I would actually build for you.

The FDA just approved Awiqli (insulin icodec) from Novo Nordisk: the first and only once-weekly basal insulin. One injection covers your baseline for the entire week. 📅

If you have type 2 diabetes and take a daily long-acting insulin like Tresiba or Lantus, this changes your daily routine into a weekly one. Fewer missed doses. Nothing to remember at bedtime. Nothing to pack for every trip. ✈️

One honest caveat: it is approved for type 2 only, not type 1 yet. And a weekly insulin is not automatically the right insulin for you. Dosing, transition, and monitoring matter, and that is exactly what I break down in the video. 🎥

Here is what almost nobody explains before they hand you the pen: obesity is a chronic condition. Treat it well, stop the treatment, and your body drifts back toward where it started. That is not the drug failing you. That is how chronic conditions work. It is exactly why we call these forever medicines.

So the "quick round before the big day" plan has a built-in expiration date. The honest version is the one I actually put my patients on: the medication, plus a protein floor, strength training, and real monitoring. I tell them all the time: the more you do for you, the less we do to you.

That is a real exchange from my conversation on DC Dermatology Skin Truths. And we were not exaggerating.

The companies that make GLP-1s do not sell them to compounding pharmacies. So I called compounding pharmacies myself and asked: is it pure, and can you prove it? The answer, every single time, was no. Eli Lilly has publicly warned that tirzepatide compounded with B12 may pose safety risks. And here is the part nobody selling you a link mentions: the real, FDA-regulated versions now cost about the same as the copies. Oral Wegovy starts at $149 a month.

Most people blame weight. Or age. But sometimes it's neither.
