Somebody asked a great question about how current blood donor shortages are going to impact the fielding of dried plasma.

I know it sounds counterintuitive, but Blood Centers have plenty of access to plasma and can manufacture/sell it in its various forms (LP, FFP, Dried Plasma). Modeling shows they can currently meet a large prehospital demand for plasma without disrupting collections.
Check out a great paper on the topic by Matthew Levy et al:

National-scale prehospital transfusion will require coordinated EMS-hospital rotation, major donor recruitment for LTOWB, and targeted investment. A hybrid LTOWB-LP strategy may provide the most feasible path to broader implementation.

A historic day decades in the making. This has huge implications for military and civilian care!
The price has not been officially released. The product should be available at the beginning of 2027.
This is licensed as a blood product, so all current Federal/State regulatory requirements will still apply.

FDA today licensed Ezplaz Freeze Dried Plasma (FDP), making it the first freeze-dried plasma product licensed for use in the United States. Ezplaz is intended for transfusion in adult patients who need plasma and for whom other plasma products are not available.

Cost remains one of the greatest barriers to implementing and sustaining prehospital blood programs. From equipment and consumables to blood products and reimbursement, every part of the program must be financially sustainable.

Join the PWW Advisory Group and the Prehospital Blood Transfusion Coalition on Thursday, July 30, at 1 p.m. CST for The Price of Saving Lives: The Economics of Prehospital Blood Administration Programs.

The webinar will feature our friends Jon Krohmer and Randi Schaefer of the Prehospital Blood Transfusion Coalition. Together, they'll explore the financial and operational realities facing EMS agencies, including what the new TRICARE Prehospital Blood Transfusion Demonstration could mean for reimbursement and future access.

MAJOR ANNOUNCEMENT & ACTION NEEDED:

TRICARE, under the Defense Health Agency/United States Department of War, has announced a 5-year PREHOSPITAL BLOOD TRANSFUSION DEMONSTRATION PROJECT to help civilian prehospital blood agencies get reimbursed for the care they provide.

There are over 8.8 million TRICARE beneficiaries in the United States (https://dha.mil/About-DHA/TRICARE-Numbers). Even if you are not located near a military installation, there is a strong chance that you take care of TRICARE beneficiaries.

ACTION:
1. Spread the word as widely as possible: the demonstration pilot kicks off on January 1st, 2027.
2. Air and Ground EMS agencies: make sure your documentation accurately meets the required elements for reimbursement.
3. Prehospital Billing Agencies: close the loop with your agencies to ensure you are ready to launch on January 1st, 2027.
4. This is a demonstration pilot. It will only be successful if claims are submitted during the project period.
5. The link to the Federal announcement with the program details: https://www.federalregister.gov/documents/2026/07/06/2026-13515/tricare-demonstration-project-for-tricare-ambulance-add-on-reimbursement-for-pre-hospital-blood.

The groundwork has been done by folks advocating for EMS; now it is up to the agencies (and their billers) to take it across the finish line.
