MRI demonstrates symmetrical abnormalities in the bilateral putamina and caudate nuclei.

Hey guys, I have another clinical vignette for you đź‘€

A 70-year-old man presents with a 2-year history of worsening balance, backward falls, slowed speech, and difficulty with executive function. Exam shows a wide-eyed stare, decreased blink rate, axial rigidity, impaired downward gaze with intact doll’s eyes, and an unsteady gait with retropulsion. Sinemet hasn’t helped. MRI reveals the classic “hummingbird sign” caused by midbrain atrophy.

Clinical vignette answer time đź§ 

This 58-year-old retired engineer’s symptoms pointed toward Multiple System Atrophy (MSA), not typical Parkinson’s disease.

The combination of:
• Severe autonomic dysfunction
• Poor response to Sinemet
• Parkinsonian + cerebellar findings
• Nocturnal stridor

…strongly suggests MSA, a progressive neurodegenerative disorder that can mimic Parkinson’s early on but has distinct clinical features.

Recognizing these red flags is critical for early diagnosis and management.
