A 19-year-old college student presents with a six-month history of worsening hand tremor, slurred speech, and difficulty with handwriting. His roommate reports increasing irritability and impulsive behavior.
On exam:
🔹 Dysarthria
🔹 Bilateral wing-beating tremor
🔹 Dystonic posturing & rigidity
Slit lamp exam reveals Kayser-Fleischer rings.
Lab work shows:
⬆️ AST/ALT
⬇️ Ceruloplasmin
⬆️ Urinary copper excretion
MRI demonstrates symmetrical abnormalities in the bilateral putamina and caudate nuclei.
