🌟 Building Community Connections for Better Care! 🌟

Today I had the pleasure of networking with the amazing Case Management team at Chesapeake Regional Medical Center (CRMC) and learning more about the valuable resources available throughout our community while sharing what Rosewell Adult Day Center has to offer their patients.

A special thank you to Tony for taking the time to introduce me to the Chesapeake Mobile Health Bus, an incredible program that brings healthcare services directly into the community through its various park-and-serve locations. The Mobile Health Bus helps increase access to care for individuals and families who may face barriers to traditional healthcare services, ensuring that everyone has an opportunity to receive support and health resources. Today, our participants celebrated National Smile Power Day with creativity, laughter, and meaningful engagement! 🌻

We spent the day spreading positivity through arts and crafts, sharing smiles, and reminding one another that even the smallest act of kindness can brighten someone's day. Our participants enjoyed creating colorful smile-themed artwork while connecting with friends in a warm and supportive environment.

At Rosewell Adult Day Center, we provide compassionate, person-centered care that supports the social, physical, emotional, and cognitive well-being of aging adults.

What makes Rosewell different is our clinical oversight. Our program is led by a Dual Board-Certified Nurse Practitioner in Family and Psychiatric Mental Health Nursing with neurology experience focused on recognizing early behavioral and cognitive changes in individuals living with mild to early moderate dementia and chronic neurological conditions.

• Mood & behavior changes
• Increased confusion

• Safety concerns

We collaborate with healthcare providers to help identify concerns early and support timely interventions when changes occur.

They may can not tell you what they like or dislike but knowing a persons dislike and likes from family helps us to develop a person centered care plan for each participant.

This participant is a well known prosecutor and guess what we are watching, Law and Order. He has a smile on his face and is engaging with it. This was after a morning walk and sitting and talking about his career as a prosecutor (I looked him up and asked him questions about what I found). This is reminiscence therapy and is a part of his person centered plan.

Our participant will never adjust to us, we adjust to them. 💙
