Thinking about explant but worried about losing volume?

For the right candidate, Dr. Landon Pryor may recommend fat transfer at the same time as explant surgery to restore softness, shape, and natural volume without replacing the implants.

Fat is harvested from an area such as the abdomen, flanks, or thighs, carefully processed using the Tulip system, and strategically transferred into the breasts.

Not all transferred fat will remain. Retention varies, and some patients may lose up to approximately 50%. For example, if more than 200 cc is transferred, the lasting volume could still be comparable to a small implant, depending on the individual patient and healing process.

The best candidates have enough donor fat, a suitable BMI, realistic expectations, and are medically cleared for surgery.

Fat transfer is considered a safe and effective option when performed by an experienced, board certified plastic surgeon using proper technique.

9 Months Post Explant Transformation

This beautiful 49 year old patient is shown nine months after explant surgery with capsulectomy, muscle repair, mastopexy, and fat transfer performed by Dr. Landon Pryor.

Explant surgery is not simply about removing the implants. Dr. Pryor carefully evaluates the capsule, the condition of the chest muscles, the remaining breast tissue, skin laxity, and each patient’s individual goals to create a thoughtful and customized surgical plan.

Dr. Pryor’s approach includes total intact capsulectomy, commonly referred to by many patients as “en bloc” removal. He also performs muscle repair when the implant pocket has stretched, weakened, or displaced the chest muscles. A mastopexy can reshape and lift the natural breast tissue, while fat transfer may be used to restore soft, natural volume without placing new implants.

The goal is not to replace one implant with another. It is to restore the patient’s natural anatomy and create the best possible result using her own tissue.

✨ Mini Lift vs. Full Breast Lift After Explant: Which Is Right for You?

Not every patient needs the same type of breast lift after implant removal. The most appropriate approach depends on your natural breast tissue, skin elasticity, ni**le position, implant size, degree of sagging, and desired outcome.

A mini lift may be appropriate for patients with mild skin laxity, minimal ni**le drooping, and a smaller amount of tissue reshaping needed. It typically involves less extensive skin removal and may result in a shorter scar. However, it is not designed to correct significant sagging.

A full breast lift, also known as a mastopexy, may be recommended for patients with moderate to significant breast drooping, stretched skin, downward pointing ni**les, enlarged ar**las, or noticeable tissue changes following implant removal. This technique allows for more complete reshaping, removal of excess skin, and repositioning of the ni**le and ar**la.

Choosing the smallest scar does not always produce the best result. An insufficient lift may leave behind excess skin, poor breast shape, or an outcome that does not align with the patient’s goals.

Every surgical plan should be based on the patient’s individual anatomy and expectations.
