The patient (and family / care partner) interview is one of theeeeee most important parts of our evaluation.

It's right up there with a thorough chart review (assuming we're lucky enough to have a *thorough chart* to review 😜)

We need to know how to establish rapport and ask the right questions... and also when to shut our mouths and listen.

So many clinicians tell me they're worried they don't have enough time to address respiratory muscle weakness (RMW) in therapy.

Which I get-- it does involve (a little) extra testing & setting up devices.

But what about managing the complications that can come from not addressing RMW?
.. The repeated (and long) hospitalizations. The pneumonias. The declining function from kinesiophobia...

As a practicing therapist myself, I get that our time is limited and caseloads are huge. But respiratory muscle training doesn't have to be complicated or time-consuming!

Once you get the hang of it, you'll see how quick-- and powerful-- RMT is.

We SLPs have GOT to embrace the grey.

(much like I've had to do with my hair since I was a tween, but that's another story 👵)

It's tempting to have a yes/no, this/that, black/white perspective on SLP things-- thickening liquids is a hot one right now.

But working with human beings with a variety of values, and in healthcare with a patchwork quilt of illnesses and outcomes... what we do is simply is never black and white.

The faster we embrace the grey, and the more we have open conversations about it, the better our patient care will be.

Here's what most SLPs, PTs, OTs, and RTs don't realize:

Inspiratory muscle weakness doesn't just affect breathing.

It creates a domino effect that impacts balance, exercise tolerance, post-op recovery, and many other important functions.

Great news: Inspiratory muscle training (IMT) can help.

Research is compelling-- IMT has been shown to:
✳️ Lower blood pressure in hypertensive people
✳️ Reduce dyspnea and improve exercise tolerance
✳️ Decrease post-operative pulmonary complications
✳️ Enhance communication

Think about your current caseload: people with chronic pain, cardiovascular disease, COPD, or neuro conditions ... how many of them might benefit from stronger inspiratory muscles?

Taking care of people with trachs was a really steep, scary learning curve for me when I was a new MedSLP.

I was terrified I was going to unalive someone (knock on wood, that's never happened).

I didn't know how to troubleshoot when someone wasn't tolerating a speaking valve.

And I kept thinking, 'what the hell does "tolerating" mean anyway?'

Fortunately, with lots of education and mentoring, those feelings are in the distant past.

Save this for later! 🫀 SLPs and the heart ...

You might know about the potential for dysphagia and dysphonia after cardiac surgery... but what else?

There are SO MANY ways the heart can affect what we do, like:
- RLN injury: voice and swallowing
- Deconditioning: swallowing, breathing, & communication
- Enlarged aorta and/or atrium: swallowing
- Cardiac diet: sodium and fluid restrictions
- Cardiovascular disease: risk for stroke & associated impairments

And remember: the heart is just one component of a large, complex network of organs and body systems. We need to consider them ALL in our care of patients, no matter what setting we're practicing in.

Because the hip bone's connected to the hyoid bone. 🫳🎤

Yes! Trachs and respiratory muscle training are often a bit of a power couple.

It can take some modifications, depending on IMT vs EMT and things like whether the person is using a speaking valve.

RMT can be a powerful tool in rehabilitating people who have been dependent on the ventilator -- I've seen fantastic outcomes in my patients in acute care, and the research shows the benefits of IMT specifically.

Learn how to do this with your patients in the course:
Breathehab: Respiratory Muscle Training for Clinicians.

Whether you're an SLP, PT, OT, or RT, you'll love this course (and your patients will love your new knowledge).

Take the course at the link in bio or comment LEARN and I'll send you the link.

Behind the scenes at Breathehab

Yep, this is me reading research at the beach.

(Don't worry - I also splashed in the waves and collected a bunch of shells with my daughter.)

This is the reality behind everything I create for you - the courses, newsletters, social media, all of it. After my day job and in between family events, I'm constantly improving my own knowledge so I can help you grow yours.

And that's exactly why you can trust what I teach. I'm not just regurgitating stuff you could Google or asking AI to spit out content for you. I'm digging into the actual research and figuring out how it applies to the real world.

Every resource I create starts with moments like this - me, a highlighter, and endless curiosity about how respiratory muscle training actually works.

Thanks for being here with me .official 💚
