How do you make sure patients truly understand all their options?

For Dr. Tokuhara, the answer is simple: lay everything out.

Some patients come in with multiple issues affecting their vision – for example, a corneal condition alongside cataracts. One approach may involve treating the cornea first, allowing it to heal, and then performing cataract surgery months later. Another option may be to proceed directly to cataract surgery for a faster recovery, understanding the trade-offs involved.

Both options must be discussed.

Dr. Tokuhara believes his role isn’t just to perform surgery – it’s to help patient’s think through the pros and cons of each path so they can choose what best fits their goals, their lifestyle, and their timeline.

“In reality, only about 1% of what I do is surgery,” he says. “The other 99% is counseling patients through their options.”

Because good medicine isn’t about pushing a procedure. It’s about helping people make the right decision for their life.

Ophthalmology is unique in medicine.

Cataract surgery is one of the few procedures where Medicare has created special billing rules for the eye. The surgery itself is covered, but some patients may also have the option to choose advanced lens implants designed to reduce dependence on glasses.

In most surgeries – like knee replacements or cardiac procedures – patients typically don’t choose between implants based on lifestyle of visual goals, and they generally aren’t asked to pay additional out-of-pocket costs for different implant options.

Cataract surgery is different.

Dr. Tokuhara believes the ethical responsibility is simple: clearly explain every option, including what insurance covers and what it doesn’t so patients can make informed decisions without pressure.

Because the right choice is the one that fits your vision, your lifestyle, and your priorities.

Not every cataract evaluation should be routine.

At Desert Vision Center, Dr. Tokuhara looks at the entire eye before recommending surgery, including both the retina and the front structures of the eye, to make sure underlying issues are not overlooked.

As the only board-certified cataract surgeon in the Coachella Valley with fellowship training in retina, he brings a broader level of insight to both routine and more complex cases.

That extra level of evaluation can make a meaningful difference in long-term visual outcomes, especially in eyes with prior surgery, retinal concerns, or other risk factors.

Because good surgery starts long before the procedure itself.

We understand that cataract surgery can feel like a big decision, especially when recovery time, transportation, and time away from daily life are concerns.

That’s why Dr. Tokuhara offers CLEAR in a Day™, an advanced approach that allows qualified patients to have both eyes treated on the same day with careful planning and modern surgical techniques.

For many patients, this means fewer appointments, one recovery period, and a faster return to balanced vision.

When it comes to eye surgery, experience matters. So does perspective.

Dr. Tokuhara’s background in both cataract and retinal disease management allows him to approach surgery with a more complete understanding of the eye, especially in cases that may require more detailed planning.

At Desert Vision Center, that experience is applied to everything from routine cataracts to eyes with prior surgery, retinal conditions, or other complicating factors.

Patients aren’t treated like numbers here. Every surgical plan is built around the individual eye and the long-term visual outcome.

Not every cataract surgery goes as planned. That’s why Dr. Tokuhara created R.E.F.O.C.U.S.™ at Desert Vision Center.

R.E.F.O.C.U.S.™ stands for Revision of Eye Function & Optical Clarity Using Surgery, a specialized program designed for patients experiencing poor visual outcomes, lens complications, iris defects, or ongoing issues after prior eye surgery.

As the only board-certified cataract surgeon in the Coachella Valley with fellowship training in retina, Dr. Tokuhara brings a broader understanding of both the front and back of the eye when evaluating these complex cases.

Many patients come to us after being told they have no other options.

Sometimes the right solution starts with a second look.

The conversation about ethics in eye care doesn’t stop with surgery.

In this interview, Dr. Tokuhara reflects on changes across the eye care landscape – including modern optometry – and the growing importance of transparency, patient trust, and recommendations that are guided by medical judgment rather than sales pressure.

At Desert Vision Center, every treatment discussion starts with the same question: What is truly best for the patient’s vision and long-term health?

Because ethical care isn’t about selling services – it’s about earning trust.
