Frontal fibrosing alopecia (FFA) is a scarring alopecia clinically defined by the progressive, symmetrical recession of the frontotemporal hairline and frequent partial or complete loss of the eyebrows. Considered a distinct clinical variant of lichen planopilaris, FFA is characterized histologically by a dense, inflammatory infiltrate targeting the upper follicle (the infundibulum and isthmus). This immune attack selectively destroys follicular epithelial stem cells in the bulge region, permanently obliterating the hair follicle and replacing it with fibrous scar tissue. The resulting band of exposed scalp typically appears smooth, pale, and devoid of follicular ostia, often accompanied by perifollicular erythema and follicular hyperkeratosis at the active advancing border.

While its precise etiology remains multifactorial and incompletely understood, FFA predominantly affects postmenopausal women, strongly pointing toward hormonal dysregulation, genetic susceptibility, and potential environmental triggers. Because the associated follicular destruction is irreversible, therapeutic management does not aim to regrow lost hair, but rather to arrest progression. Early dermatologic intervention is essential to prevent permanent, disfiguring hairline recession.
