Can you breastfeed if you have HIV? New policy guidelines say yes, in some cases it's safe.

A policy change in May 2024 by the American Academy of Pediatrics (AAP) updated recommendations for parents with HIV. Parents with HIV can safely breastfeed their babies if they are consistently taking antiretroviral medications that effectively suppress the virus.

This marks a significant shift from previous guidelines, which had discouraged breastfeeding by parents with HIV in the U.S. since the 1980s.

A full room does not always mean she feels seen or heard.

There can be family visiting, providers checking in, friends asking about the baby, and people offering advice, while no one stops long enough to really see her.

Support is more than being present. It is noticing. Listening. Believing her when she says something feels different. Making sure she has food, rest, care, resources, and space to speak honestly without judgment.

As birth workers and maternal health advocates, I think we have to keep asking ourselves: Are we simply surrounding mothers, or are we creating communities where mothers actually feel supported?

What does meaningful postpartum support look like to you? I would especially love to hear from doulas, midwives, lactation professionals, nurses, community health workers, and fellow advocates.
