You can age faster on the inside than the outside.

Two people the same age — same lifestyle, same lab "normals," same outward health — can have very different biology underneath. The variable is often chronic, low-grade inflammation.

Unlike the acute inflammation that helps you heal from an injury or fight an infection, chronic inflammation does not produce pain. It does not show up on a routine physical. It does not announce itself in symptoms most patients would notice.

But it is doing something.

Chronic inflammation has been linked to nearly every major age-related condition — including atherosclerosis, hypertension, insulin resistance, cognitive decline, and heart failure. In cardiovascular medicine, it's what makes plaque dangerous. Plaque alone is common. Inflamed, unstable plaque is what ruptures.

The good news: inflammation is one of the most measurable processes in the body. Biomarkers like hs-CRP, IL-6, homocysteine, GGT, and emerging composite scores like iAge make it visible. And once it's visible, it's something we can address — through nutrition, sleep, metabolic health, stress regulation, environmental factors, and when appropriate, medical therapy.

Your cholesterol number isn't the whole story.

For decades, LDL has been treated as the headline metric in cardiovascular risk. It's the number most patients know, most doctors quote, and most decisions are built around.
It's useful. It's also incomplete.

ApoB measures something different — and arguably more important. Every particle in your bloodstream capable of driving plaque carries exactly one ApoB protein on its surface. So when we measure ApoB, we're not measuring how much cholesterol you have. We're counting the actual particles depositing it into your arteries.

This distinction matters most in the patients who look "fine" on a standard panel. Normal LDL with elevated ApoB is more common than most people realize, particularly in those with insulin resistance, elevated triglycerides, or metabolic dysfunction. Without ApoB, that risk stays invisible.

ApoB has been consistently linked to cardiovascular events in large-scale studies — and unlike many advanced markers, it's modifiable through nutrition, metabolic health, body composition, and when appropriate, medical therapy.

One blood test. Once in your lifetime.

That's all it takes to measure Lp(a) — a genetically inherited lipoprotein particle that independently raises cardiovascular risk, often in patients whose standard cholesterol numbers look reassuring.

A standard cholesterol panel doesn't measure it. It's genetically set at birth. And it's been linked to earlier heart attacks, stroke, and aortic valve disease — independent of LDL, blood pressure, or lifestyle.

Most people will never have it ordered unless they specifically ask.

Knowing your Lp(a) doesn't change your genetics — but it sharpens the rest of the prevention picture. When one risk factor is fixed, every modifiable one matters more.

Most heart attacks don't come with a warning.

They come with a history — quietly written over years, sometimes decades, before the first symptom ever appears.

Plaque does not announce itself. Inflammation does not announce itself. Insulin resistance, vascular aging, and elevated ApoB do not announce themselves.

By the time the body sends a signal loud enough to notice, the underlying process has often been in motion for a very long time.

This is the reason modern preventive cardiology has moved beyond waiting for symptoms. The tools available today — advanced biomarkers, imaging, metabolic and inflammatory markers, genetic context — allow us to read that history while it's still being written, not after it has reached a critical point.

Prevention is not about fear. It's about visibility.

The Mediterranean diet is the most extensively researched dietary pattern we have. The evidence is consistent across cardiovascular disease, type 2 diabetes, certain cancers, and overall mortality.

When researchers look at what's actually driving the results, two patterns keep surfacing.
The foundation is plants. Vegetables, in volume, daily. This is the part of the pattern that cannot be substituted, supplemented, or worked around.

Fat-soluble nutrients require fat for absorption. A study referenced by Dr. Artemis Morris in this week's episode found that lycopene absorption from tomatoes increased meaningfully when consumed with olive oil. The same principle applies to carotenoids in leafy greens, vitamin K, vitamin E, and a number of polyphenols.

Eating vegetables without extra virgin olive oil leaves a significant portion of their nutritional value on the plate.

This is one of the reasons the Mediterranean pattern outperforms reductive approaches that isolate single nutrients. The components are designed to be eaten together.
