At Moonlite Transitions dba, Moonlite Geriatric Care Management, we work with at-risk seniors, to provide assessments and care plans at low cost (no cost) to the senior or their families. Sometimes this means hiring in home care workers to ensure the safety of the senior to allow them to remain in their home. Sometimes the plan will be placement into

community based care facilities. We provide `A Better Way` for seniors and their loved ones to maneuver their way through the system of senior care options and providers with dignity and respect. Before you get lost in the maze, call one of our qualified Geriatric Care Managers who can easily guide you through the process.

WhINE ABOUT YOUR AGING PARENTS: BREW UP A BETTER CARE PLAN IS BACK!!! Please join us for the first since Covid hit in 2020. We will be discussing all the things you need to create the perfect care plan for your aging loved ones and for yourself to help your kids before something happens. We will have our very own Medina Van Alstyne directing us in painting a masterpiece to take home. No charge; just come and have some snacks,, paint, and learn how to be prepared.
