A mother is on trial right now for the unthinkable. And I cannot stop thinking about how many people failed her long before that day.

You may have seen the Lindsay Clancy case in the news. A mother, a former labor and delivery nurse, in the grip of what her defense describes as severe postpartum psychosis, who killed her three children and then tried to end her own life.

I am not here to try her case. A court will do that.

I am here to talk about everything that comes before a mother reaches a moment that dark. Because postpartum psychosis does not appear out of nowhere. It grows in isolation. In exhaustion. In the spaces where a struggling mother is left alone, unsupported, and unseen.

And we leave mothers alone constantly.

We have built a culture that hands a woman a newborn and expects her to bounce back. To host visitors days after birth. To cook and clean and smile and post the highlight reel. To "get back to normal" as fast as possible.

What normal? She will never be who she was before. She is a whole new person after every single birth. That is not a problem to fix. It is a transformation to honor.

This is why postpartum planning is a core part of my care. Not an afterthought. A cornerstone.

Long before the baby comes, I sit down with families and we make a real plan. And I bring the husbands and partners into that conversation directly, because they are essential.

Here is what I tell them.

In those early weeks, her only jobs are to rest, to recover, and to feed the baby. That's it. Everything else falls to the people around her.

Bring her meals in bed. Care for the older children. Handle the household. Guard the door. Limit the visitors, because your growing family's early days are sacred and intimate, not a receiving line. Line up real postpartum help, the kind that washes your dishes and folds your laundry, not the kind that shows up to hold the baby while mom serves them coffee.

Protect her space. Protect her rest. Protect her.

Scripture understood this long before modern medicine caught up.

There are ancient rhythms of postpartum rest and recovery woven right through the Bible, seasons of setting apart, of protection, of not rushing a mother back into the world before her body and soul have healed. We were never meant to do this alone or in a hurry.

Postpartum should be beautiful. Supported. Sacred.

That is always my goal for the families I serve. Not because a beautiful postpartum prevents every tragedy, it doesn't, and mental illness is real and deserves real treatment. But because so many mothers are suffering quietly in a culture that left them isolated and called it normal.

We can do better. We have to.

We have heard time and time again, how unprepared moms feel going into a hospital birth. How things quickly escalate and they end up in a situation they had not planned for. How they feel pressured to comply with policy even though they were told they could labor their own way.

Enough is enough ladies! You have to advocate for yourself more than ever. Going in knowing the statistics, what the risk/reward factor is, and how to pivot when needed is incredibly important to both you and your baby!

Prep really should start even before you're pregnant but it's never too late to educate yourself!

We are here to have some of those honest conversations and give you the tools you truly need to set yourself up for the best possible labor and delivery!

Who is the lead trainer for the Breathing through Birth Academy Doula/Birth Coach Program - Kimberly Oen

Kimberly is a Certified Doula through the State of Ohio Nursing Board and was the 3rd Doula to be certified when the program launched in October 2024 and currently the only Certified Doula in Auglaize County. She supports Shelby, Auglaize, Mercer, Allen and Champaign Counties but also has virtual Doula clients in Hancock, Darke, Miami, and Defiance Counties.

She trained under Birth with Spirit and Childbirth International as both a Doula and Lactation Counselor and has been offering doula services since 2020 but has been in the birth world since 2008. She is also a Placenta Encapsulation Specialist.

Kimberly is President of Reclaimed Motherhood, a non-profit organization supporting women and babies. She is a Board-Certified Mental Health Coach through the American Association of Christian Counselors with a master’s in human services counseling. Other training in the birth world includes Registered Prenatal Yoga Teacher through Yoga Alliance, Postpartum Doula (Wise Woman Way of Birth – Gloria Lemay).

She has been a Certified HypnoBirthing Childbirth Educator since 2021 and is also trained through Fit for Birth as a Pregnancy Exercise Specialist and Diastasis Recti and Core Consultant. In 2025 she completed her Doctorate as a Traditional Naturopathic Doctor and Integrative and Functional Medicine Practitioner. A professional student and lover of knowledge, she is currently training as a Fertility Health Coach.

Her faith and wellness journey began the moment God gifted her with a son with Cystic Fibrosis. At his birth, it was considered a fatal illness with a life expectancy of 18 years old. Today at 29 he is alive and thriving with a 47% lung function. She owes all that she is and all that she has to God. She is committed to supporting mothers throughout their journey to motherhood and beyond.

Did you know that whether a baby receives human milk or not affects the colonization of their gut? Babies fed human milk have a lower (meaning more acidic) gut pH of 5.1 to 5.4 in the first six weeks of life which reduces the number of E. coli, streptococcal, and clostridia bacteria in their guts among others. Babies fed formula have a higher gut pH of 5.9 to 7.38 which allows for the growth of more harmful bacteria, especially Enterococci. Even a single bottle of formula given to otherwise breast-fed baby will disturb the gut flora bacteria, and raise the pH and babies gut may never recover even if feeding with human milk is fully resumed. The baby fed formula, even a little, will tend to have a gut like a formula fed baby, not like a baby given human milk. Learn more in my upcoming Doula and Birth Coach training. We begin August!

There's more data on the benefits of women supporting women?? 🥳

It's actually sad that Ob-Gyns need guidance on how to get along with Doulas who only have the mothers best interest in mind. For my clients, postpartum is just as important as the pregnancy and birth. A mother spends 15 minutes at their OB-GYN 6-week check up where they are told they can now have s*x and resume normal activity. There is no normal, only a new-normal. As your doula, I keep the relationship open for as long as mom needs support. THIS is critical for all new moms to have that support after baby.

It's a collaboration. I do not work for the hospital, nor the insurance. I work for the mom. Obstetric care has become organized in a way that women need advocates, mediators, and interpreters just to receive routine maternity care.

One of the hardest parts about infant tongue tie is that there isn’t a single symptom that proves a baby has one. In fact, almost every symptom people associate with tongue tie can also happen for completely different reasons.

That’s why tongue tie should never be diagnosed from a checklist alone. It’s diagnosed by looking at anatomy, tongue function, feeding, growth, and the whole clinical picture.

That said, there are definitely patterns we see more often in babies whose tongue mobility is restricted.

Here are some of the symptoms that can be associated with tongue tie, along with some of the other things that can cause the exact same problem.

🍼 Painful breastfeeding

Many parents first seek help because breastfeeding hurts. A restricted tongue may not lift or extend well enough to maintain a deep latch, leading to ni**le compression and pain.

But pain doesn’t automatically mean tongue tie.

Pain can also be caused by:
• Shallow positioning
• Engorgement
• Vasospasm
• Dermatitis or skin conditions
• Incorrect pump fl**ge sizing
• Oral tension
• Prematurity
• Flat or inverted ni**les
• Oversupply or forceful letdown
• Incorrect latch mechanics unrelated to tongue mobility

👶 Difficulty latching

Some babies struggle to stay attached to the breast. They may repeatedly slip off, lose suction, or have trouble maintaining a seal.

This may happen with tongue restriction, but it can also occur with:
• Prematurity
• Low muscle tone
• High muscle tone
• Birth interventions
• Torticollis
• Oral tension
• Sleepiness from jaundice or illness
• Maternal engorgement
• Large ni**les compared to a very small newborn mouth

💥 Clicking while feeding

Clicking often means the seal around the breast or bottle is repeatedly breaking.

Tongue tie is one possible reason.

So are:
• Fast milk flow
• Bottle ni**les with flow rates that are too fast
• Positioning issues
• Oral tension
• Learning to coordinate sucking in the early weeks
• Fatigue during feeding

🌬️ Lots of gas

Parents are often told that gas automatically means tongue tie because babies swallow more air than normal. While excess air intake can happen when a baby loses suction, gas is incredibly common in young babies regardless of tongue function.

Gas can also be related to:
• Normal newborn digestive development
• Crying
• Fast milk flow
• Bottle flow that’s too fast
• Feeding position
• Overfeeding
• Normal infant behavior

💩 Green, frothy stools

Some babies with inefficient feeding have green, frothy stools.

But this can also occur with:
• Oversupply
• Fast letdown
• Normal variation
• Viral illnesses
• Formula changes
• Temporary digestive changes

😢 Long or frequent feedings

If a baby can’t transfer milk efficiently, they may spend a very long time at the breast.

However, long feeds can also happen because:
• Low milk supply
• Prematurity
• Sleepiness
• Cluster feeding
• Comfort nursing
• Slow milk ejection
• Normal newborn behavior during growth spurts

⚖️ Poor weight gain

Poor milk transfer from limited tongue mobility can absolutely affect growth.

But poor weight gain deserves a broad evaluation because it can also result from:
• Low maternal milk supply
• Medical conditions
• Heart or lung disease
• Metabolic disorders
• Neurological conditions
• Incorrect formula preparation
• Feeding management challenges

🤱 Constant feeding

Some babies seem hungry all day because they aren’t transferring enough milk efficiently.

But frequent feeding can also simply mean:
• Normal newborn behavior
• Cluster feeding
• Growth spurts
• Feeding to regulate emotions
• A baby with naturally smaller, more frequent meals

💔 Damaged ni**les

Lipstick-shaped ni**les, blisters, cracks, and compression lines are often discussed with tongue tie.

These can certainly occur with restricted tongue movement.

They can also result from:
• Poor positioning
• Pump trauma
• Incorrect fl**ge sizing
• Skin conditions
• A shallow latch without tongue restriction

🍼 Bottle feeding problems

Tongue tie doesn’t only affect breastfeeding.

Some babies struggle with bottles too. They may leak milk, collapse the ni**le, click, tire quickly, or take a very long time to finish a bottle.

But bottle feeding difficulties can also be caused by:
• Bottle flow rate
• Bottle shape
• Oral motor immaturity
• Prematurity
• Sensory differences
• Reflux
• Feeding aversion
• Neurological conditions

😴 Falling asleep constantly during feeds

Sometimes babies work so hard to transfer milk that they tire out before getting a full feeding.

But sleepiness can also be related to:
• Jaundice
• Prematurity
• Illness
• Medications
• Normal newborn sleep patterns

💛 So what does all of this mean?

The biggest takeaway is this:

Symptoms are clues, not conclusions. A baby with ten symptoms may not have a tongue tie.

A baby with only 1-2 symptoms may have a significant restriction.

This is why experienced feeding assessments are so valuable. Rather than asking, “Does my baby have this symptom?” we ask, “Why is this symptom happening?”

Sometimes the answer is tongue tie.

But sometimes it’s positioning or milk supply or body tension or bottle mechanics.

And often, it’s a combination of several of these factors working together.

The goal is to understand why feeding isn’t working as well as it could, so families receive the treatment that actually matches the problem.

Tongue tie is one piece of the puzzle, but it’s never the whole puzzle.
