This data seems counterintuitive. You’d think anyone starting a GLP-1 would be moving more, not less. But the numbers don’t lie, and they’re showing a weird dichotomy in GLP-1 care right now.
New research out of ENDO 2026 tracked real Fitbit data on almost 2,000 people with obesity who started a GLP-1. Daily steps dropped from 5,047 to 4,487. Moderate to vigorous activity dropped 21%, from 28 minutes a day down to 22. Men and people with joint or muscle pain had the steepest declines.
Yet, here is what I see in my practice. My patients steps and exercise are going up. My suspicion with this study is that these people weren’t getting the same 1:1 care and accountability that my patients are getting. And we already know from other research how much that support matters for actually keeping the results you’re working for.
GLP-1s work. What you build around them is what decides whether you keep muscle or lose it along with the fat. That’s why my patients get body composition tracking, a real activity plan, and accountability built around the medication instead of separate from it.
