Same needle. Completely different job.

Uphill is fine. Coming down hurts.

You get up the hill with your lungs burning and your legs working. Then you turn around, and halfway down the knees start talking. That night the quads are sore in a way the climb never made them.

So you decide you are out of shape, and you go climb more.

Here is the part that actually matters. Going up, your muscles shorten to lift you. Going down, they lengthen while they hold you back. That second job is braking, and it is the highest force your legs produce all day.

The quad holds the knee. The calf holds the ankle. The hip and low back hold your trunk over a falling foot.

A climb tells you it is hard, so you pace it. A descent feels easy while the load quietly stacks up. That is why the way down is the part that costs you two days.

Every flight of the Filbert Steps. Every mile of the Dipsea descent. Everyone trains the engine. Nobody trains the brakes.

So we do not start with your quads. We look at what should be sharing the braking. An ankle that will not bend forward over the planted foot hands the whole job to the knee, which often shows up as pain at the front of the knee on stairs. A hip that will not hinge back hands it to the low back, which is why a long descent can finish in your back rather than your legs.

Find the joint that stopped helping. Restore the motion it lost. Then load the brake back on purpose, before the next descent.

Right hip, right low back, month after month. You stretch that side, you roll that side, you sleep on the other one to give it a break. Two weeks later it is back in exactly the same spot.

At some point someone told you one leg is shorter, and you left it there.

Here is the part that actually matters. A leg can read short on the table for two completely different reasons, and only one of them is about the bone.

A structural difference is the bone itself being shorter. It measures the same on any day and in any position, and it does not change when the pelvis is moved.

A functional difference is a pelvis that has rotated. Turn one half of it forward and that leg is pulled up, so it reads short lying down. The bone is exactly the same length it always was.

Those two look identical on the table and need opposite plans. One is usually managed with a lift in the shoe. The other is about restoring the motion the pelvis lost, then making sure the position holds once you load it again.

It also explains why it is always that side. The side that compensates absorbs the extra load on every step and every stair, which is why the pain is never random and never switches.

This is why guessing in front of a mirror does not settle it. Standing, the answer changes with how you shift your weight. Face down with the heels lined up, then measured again after the pelvis has been moved, it starts to mean something.

Six weeks of rest. Back in three days.

You rested the elbow. You never rested the grip.

Here is the part nobody explains. The muscles that hold your wrist steady all anchor onto one small ridge on the outside of your elbow, and they fire every time you close your hand around something.

A mouse. A coffee cup. A doorknob. A steering wheel. A grocery bag carried palm down.

None of that involves bending the elbow, which is exactly why six weeks off the racket and off the weights missed it. The tendon counts total load rather than effort, and the hours are long. You rested your elbow for six weeks and gripped something roughly every minute of it.

That is also why it felt fine by week two and came straight back on the first heavy bag. Rest lowers what the tendon can tolerate while you wait, so you return to the same demand with less capacity than you started with.

So the answer is not more rest.

First we find which grip and which angle actually reproduces your exact spot, and we check the wrist and shoulder too, because the forearm rarely works alone. Then we treat the tendon directly with Graston, ART or shockwave. And we load it on purpose, gradually, which is the part your desk will never do for you.

The scan showed wear and tear. So you stopped running.

Nobody actually said stop. But you heard stop.

Here is what that picture could not tell you.

Radiology language is descriptive rather than final. Degenerative changes is a general term for normal age related change in discs and joints. Disc desiccation means the disc holds less water than it used to. Facet arthropathy means wear in the small paired joints at the back of each spinal level.

All three describe what your tissue looks like right now. None of them describe how much it hurts.

This is the part nobody explains. Scan a room full of people with no pain at all and plenty of them show the same changes. Common findings. No symptoms.

So the report describes your parts. It does not explain your pain.

There is a second problem. A still image is taken lying down and holding motionless. It says nothing about how your back behaves when you load it, twist it, carry something awkward, or run downhill on it. That part has to be tested in person.

Until somebody does, a report gets read as a limit on your life when it was only ever a description of one moment.

In the room we check four things. Whether the level on the page matches where your symptoms actually are. Which segments still move. What happens when we load the joint and the nerve together. And what has quietly taken over the work when one level stopped sharing it.

None of that needs the scan repeated, and it takes about ten minutes.

Twenty minutes of ice. Then a hot shower, because the ice did nothing.

Then a heat pack at your desk, because someone at work swore by heat.

Three days later it still hurts, and now you think you picked the wrong one.

You didn't. That was never the decision that mattered.

Here is what ice and heat actually do.

Cold slows the signal coming out of an irritated area and limits swelling in something fresh. For about an hour the place feels calmer than it did.

Heat brings blood in and makes stiff tissue more willing to lengthen. For about an hour the place moves more easily than it did.

Two different routes to the same destination. Both of them hand you a window where your body will let you do more than it would have.

That window is the real product, and what you do inside it is the treatment. Waste it and you are back where you started by the evening.

The practical version is simpler than the internet makes it. If it is fresh, warm to touch and puffy, reach for cold, then move it gently. If it is stiff rather than swollen, the kind that loosens as the day goes on, reach for heat before you move, not instead of moving. If you genuinely cannot tell, pick whichever feels better today.

Twenty minutes at a time, and always a layer of cloth between the pack and your skin. Skip both if the skin there has no normal sensation, or if circulation problems have been flagged for you, because numb skin cannot tell you it is being burned or frozen.

All of that is for the flare. Neither pack touches the reason it flares.

In the room we go after the reason. Which joints have stopped moving, what is carrying the load instead, and what your last three flares had in common. Treat that and you stop picking a side every few weeks.

Three weeks into training for Bay to Breakers, the same spot on your shin lights up.

You rest it. You ice it. You restart slower. Same spot, same week, every single time, and it always seems to reappear right as training ramps up.

Shin splints are not one big injury flaring up. It is the muscle running along your shin bone getting stuck to the tissue around it, so every stride pulls on a spot that cannot glide the way it should. Stretching pulls on that stuck spot. It does not release it.

Six months of planks and your back still went reaching for a laundry basket.

Here is the part nobody explains.

A plank is a hold. It trains you to resist movement while nothing is moving. That is a real skill, and it is also the one situation your back is rarely in when it goes.

Your back goes at the transitions. The bend, the twist, the reach, the lift off the floor. The moment the demand changes is the moment your low back needs cover, and that moment is brief.

Your deep core is supposed to switch on a fraction of a second before you move. In a back that keeps flaring, it switches on after. By the time it arrives, your low back has already taken the movement on its own.

That is a timing problem rather than a strength problem. A muscle can be strong and still arrive late, which is why the seconds kept going up and your back kept going anyway.

It is also why how long you can hold tells us very little on its own.

In the room, we move your hip and your low back one at a time and watch what switches on late. We look at whether your hip and mid back give you enough range to move without dumping the whole load into your lower back, and whether you can breathe while you brace. Then we work on the moment it actually fails rather than the position it never fails in.

A deep breath shouldn't stab.

You felt it reaching into the back seat. Now every yawn finds it. Every deep breath finds it. Laughing finds it. So you have been breathing shallow for four days and sleeping on one side. At 1am you searched it, and every result was about hearts and lungs.

Here is the part nobody explains.

Your ribs have joints. Twenty four of them, where each rib meets your spine at the back, and most also meet the breastbone at the front. They glide every single time your chest expands.

When one stops gliding, breathing becomes the thing that finds it. That is why it is sharp, why it is one small spot you can cover with a fingertip, and why resting it changes nothing. You cannot rest a joint that has to move every time you breathe.

It is also why it behaves differently from a pulled muscle. A strained muscle tends to be a broader ache that eases once you stop moving, and it usually fades over a week or two. A stuck rib joint stays pinpoint, bites on a deep breath or a laugh, and tends to plateau and sit there.

In the room, we find which one. We test each rib and the segment it attaches to, one at a time, to work out which one stopped gliding and which way it will not go. The guarding muscle usually has to settle before the joint will move, so soft tissue work comes first, then the joint itself. Then we watch how your ribs and mid back move on a full breath, because a joint that gets its range back still has to use it.

Then we look at why it got stuck in the first place. Often a mid back that has not rotated properly since your desk became a laptop.
