Santa Cruz Community Health Driven by a 51-year commitment to health care as a human right, Santa Cruz Community Health is a leading nonprofit provider of comprehensive health services.

We operate: Live Oak Health Center, SC Mountain Health Center and SC Women's Health Center. Through the Santa Cruz Women’s Health Center and the East Cliff Family Health Center, SCCHC (a Federally Qualified Health Center) provides comprehensive primary care services in English and Spanish to all ages, genders, ethnicities, abilities and sexual identities and orientations, regardless of their ability to pay. Seeded by the Santa Cruz Women’s Health Center, SCCHC is driven by its originator’s 40-year commitment to social justice and access to health care as human right. Its mission remains constant: to provide quality health services and advocate the feminist goals of social, political, and economic equality. SCCHC’s health centers deliver high-quality care through compassionate, dedicated medical providers, health educators, and administrative staff. SCCHC’s clinics offer primary care, pediatrics, integrated behavioral health, prenatal care and education, chronic disease management, health insurance enrollment, and more.

Please only make an appointment if:
➡️You already received your Medi-Cal renewal packet in the mail, or
➡️Your renewal due date is less than 45 days away.

You can also walk in without an appointment. You may have to wait if there isn’t an open time slot.

What to bring: Your Medi-Cal renewal packet or notice, if you received one, and any documents requested in your renewal notice. Don’t have everything? Come anyway. Staff can help you understand what may be needed.
