The National Association of Certified Professional Midwives (NACPM) issues this brief to articulate our opposition to proposals that would restrict or eliminate birthright citizenship in the United States. It documents the direct and cascading consequences such policies would have for maternal, infant, and family health — and for the perinatal care systems, community midwives, and public health infrastructure that families depend on at the most vulnerable moments of their lives.

When discussing Vitamin K options with expecting parents I present all of the the options: do the routine shot at birth, oral vitamin K, or declining. We go over risks. I explain that the risk overall for vitamin K deficiency bleeds (VKDB) is low, but that we don’t have specific risk factors or signs to know which babies are more likely to experience this severe complication and that there are typically no signs that the baby is sick until they are critical. When parents ask about the oral drops I bring up the point that is made later in this series - that you have to keep on top of the doses for it to provide the protection against VKDB that you see demonstrated in studies and that in the postpartum period is can be hard to remember to eat and/or shower so sticking to the drops schedule can be challenging and often parents unintentionally skip a dose or just stop after doing one or two doses. What that means is that the parents feel like they have protected their baby but in reality they may as well have declined all forms of vitamin K.

I am supportive of parents and their choices - they know what is best for them and their babies - but I am also a big fan on informed consent and parents are not making informed choices if they don’t understand the potential risks and benefits of *all* options available to them.

The oral route is not as effective as the shot, but does offer more protection against VKDB than doing nothing however you must *commit* to following the dosage scheduling and you need to know that it isn’t as effective as the shot in preventing VKDB.

On the week’s edition of Feathered & Furry Friends: Gimli 🐐

Gimli joined the family four years ago as an Emergency Pasture Buddy for our mustang, Mariah, after the sudden loss of her long-time herd mate. Gimli was a one year old buck that needed to be re-homed due to behaving like a buck does around a field full of does.

His former owner said she went from having 13 open does to 13 pregnant does in roughly 24hrs after rescuing him from a neglect situation. Unfortunately no fence was going to keep him from what he felt was his sole purpose on the planet and he was already too big for her ladies (meaning kidding could present major challenges). Gimli has zero interest in humans, dogs, cats, the horse, or any other creature save for female goats and he spends his days foraging in our pasture, eating weeds and other unpalatable plants, and refusing the nice expensive hay we buy for him.

He enjoys being left TF alone and carefully crafting his signature Virile Male Goat scent which carries across the property on the slightest breeze, ensuring that all who dwell here knows that there is a true stud of a man just waiting for the right doe (or really any doe) to come along. He hates socializing, me, you, and anything not shaped like a female goat.

Pickles, our Tuxedo Tom (sans 🍒), is the featured Feathered & Furry friend of the week 🐈. Pickles - named after the drummer (IYKYK) - was a Craigslist Special listed as a “Free Minx” kitten due to his lack of a tail. When we picked him up he was dehydrated, underweight, and crawling with fleas. He was also the cutest little ball of fluff you could possibly imagine.

Pickles lives up to his name and is a quirky Agent of Chaos that bangs around the house at all hours looking for fresh eggs to roll off the counter and forgotten half-filled cups of water to knock over. My husband once described him as “a cat that looks and acts like it was accidentally run through a Maytag dryer” and I don’t have anything solid to argue that observation with, especially since he refuses to drink water in a normal “cat” way and will only drink water that he scoops up with his paw and licks out of his toebeans or that dribbles off the top of his head and into his open mouth when he sticks his noggin under the sink tap.

He loves being weird, people-watching while seated in the kitchen sink, getting his back and shoulders stretched like a heretic getting pulled on the Rack, stealing pizza off your plate, and bird watching. He hates “normal”, seeing a fresh egg sitting on a counter, getting wet, boops, and watching me fill out government forms (I could be stretching him instead).
