The parasternal short-axis view allows for visual assessment of the leaflets and commissures. Commissural fusion is best seen in this view. Pseudohypertension in elderly people.
🫀 It is associated with increased density of the arterial wall of the brachial artery (characteristic of elderly and senile age), which leads to the fact that it is impossible to adequately, completely compress this artery during the measurement of BP. When we pump air into the cuff, we must completely compress it. If we cannot completely compress it - then BP is not measured. This is what happens with pseudohypertension. The presence of pseudohypertension, associated with increased density of the arterial wall of the brachial artery, can be checked using Osler's sign. It consists in tracking the presence of a pulse on the radial artery after the air has been pumped to the maximum into the cuff. If there is no pulse - it means the artery is completely compressed and the pressure is measured adequately. If the pulse persists - it means that the artery remained patent and BP is not measured.
