The reality is many families are overwhelmed, emotional, and unsure where to turn when a loved one is preparing for discharge. The fear of another hospital stay is real — and having the right plan in place matters.

That’s where I come in.

My name is Traci Gerolstein, and I serve as a trusted Family Support Advocate for seniors and their families throughout Pinellas, Pasco, and Hillsborough counties. I meet families where they are — whether bedside in the hospital, at rehab, or simply during one of life’s difficult transitions — and help guide them toward the best care options for their loved one’s unique medical, financial, and location needs.

Together, we explore solutions that provide safety, support, dignity, and a better quality of life. From independent living and assisted living to memory care and additional support services, I help families navigate these important decisions with compassion and clarity.

Most importantly, my services are completely free to families, allowing us the time needed to create a thoughtful plan for success — not another crisis.

Fabulous and informative networking breakfast hosted by Harborwood Health Center in Clearwater!

It was wonderful connecting with so many dedicated healthcare professionals, and even more exciting to learn from our guest speaker with Vivo Healthcare about their innovative Ultramist Therapy technology. This advancement is truly changing the game in wound care, helping wounds heal more rapidly and effectively.

This is especially vital for our senior population, where even a simple wound can become a long, challenging process that may not fully heal. Seeing real solutions that improve outcomes is incredibly encouraging.

Ultramist Therapy is proving to be effective for:

My heart is still smiling 💛

I had the absolute privilege of serving this sweet couple and helping them find a beautiful senior living community to truly call home. Since the move, these lovebirds are now enjoying an all-inclusive senior living lifestyle,and on my very first visit back, I found them in the promenade area, happy hour in full swing, singing and laughing with their new friends!
The first words out of their mouths to me were:
“WE LOVE IT HERE!”

Moments like this are everything. I feel so much joy and pride knowing I was able to be part of a successful, positive transition for this family. When I say it takes a village, I truly mean it. I am so grateful to partner with communities that jump in, care deeply, and work together to make moves as smooth and stress-free as possible.

A huge thank you to the amazing team at Wickshire/Colliers at Countryside and to Elizabeth Khoel for being such a true team player and helping make this move happen seamlessly 🙌

As we step into a new year, we want to pause and speak directly to the families walking the difficult journey of Dementia or Alzheimer’s. 💙

If 2025 feels overwhelming, know this: you are not failing, you are caring. This season may call for new choices, new conversations, and sometimes new levels of support, and that’s okay.

Gentle advice for the year ahead:
1. Take things one day at a time
2. Give yourself permission to ask for help
3. Remember that loving someone also means planning for their safety, dignity, and quality of life
